Biochemical markers of bone, cartilage and synovial tissue metabolism in osteoarthritis

Objectives : to investigate serum levels of osteocalcin (OC), cartilage oligomer matrix protein (COMP) and precollege III amino terminal peptide (PIII NP) as biochemical indices of bone, cartilage and synovial tissue turnover in knee osteoarthritis (OA) patients and to assess their values as markers of disease severity.

Methodology : serum levels of OC, COMP and PIII NP were measured in 20 knee osteoarthritis patients and compared to the levels in the sera of 10 healthy controls.

In osteoarthritis patients, the biochemical markers were correlated with Western Ontario and Mc Master (WOMAC) index for the quality of life and particular disabilities and with Kellegren and Lawrence (K / L) x-ray grading and radiographic evaluation of joint space narrowing (JSN).

Results : there was a significant decrease in serum OC levels and a significant increase in S-COMP and S-PIII NP in osteoarthritis patients as compared to the control group.

There was an inverse correlation between S-OC and WOMAC index, K / L x-ray grading and JSN score.

As regard S-COMP, there was a significant positive correlation between its levels and K / L x-ray grading and JSN score.

A significant positive correlation was also found between S-PIII NP and WOMAC index, K / L x-ray grading and JSN score.

Conclusion : serum OC, COMP and PIII NP levels can reflect disease severity in knee osteoarthritis patients.
