Background : Diabetes mellitus and its complication is one of the major health problems we study the prevalence of one of these complications; diabetic nephropathy and find out the follow up in our clinical practice in Sudan Objective: the main objective is to evaluate the prevalence of diabetic nephropathy among Sudanese patients in diabetes clinics in Khartoum Sudan in Omdurman Teaching Hospital Populations: during the period January–December 2008 diabetic patients with type 1 and type 2 were selected from patients attending the outpatient diabetes clinic in Omdurman teaching hospital Methodology : two hundred and thirty adult type 1 and type 2 diabetic patients were studied using simple, direct, questionnaire blood sample and urine sample for each patient were examined for renal profile and proteinuria Results : Regarding to renal and urine examination during the follow up in the referred clinic according to the; duration, age, gender and type of diabetes mellitus When the urine examined it is either examined for Sugar and acetone or none.

None of patients was diagnosed as having diabetic nephropathy or seen by a nephrologists, there is no regular examination for urine and rarely tend to have urinary examination or determination of renal profile The results were as follow : 37.4 % had apositive urine for microalbuminuria, 12.6 % were positive for macroalbmin while 50 % had negative results according to proteinuria 33 % had an abnormal results to urea estimation, while 76 % were normal 34.3 % had an abnormal result of creatinine, while 79 % had a normal level of creatinine, according to the uric acid, 27 % had hyperuriceamia, while 37 % had a normal result Result of microalbuminuria: 48.1 % were type 1DM , 40.9 % were males, 48.1 % were having DM for 21 -35 years, 41.8 % of them were in the age group 51-60 years.

Result of macroalbminuria:16.0 % were type 1 DM, 15.1 % were males , 20.0 % were having DM for 21-35 years, and 32.3 % were in age group more than 61 years.

Result of hyperureamia : 39.8 % were males, 34.6 % were type 1DM, 40.0 % were having DM for 10-20 years and 62.5 % of them in the age group 20-30 years Result of creatinine: 46.2 % were males , 39.5 % were type 1 DM, 38.4% were having DM for 10-20 years and 87.5 % of them were in the age group 20-30 years Result of hyperuriceamia:37.0 % were type 1 DM, 34.4 % were males, 36.0 % were having DM for 21-35 years, and 41.9 % of them were in the age group of more than 61 years Conclusion : Large number of our patient have evidence of a diabetic nephropathy and the majority of them are type 1 DM, having DM for 10-20 years, in the age group 20-30 years and more than 61 years and most of the patients were males and the clinical practice in our diabetes clinics in Sudan have no follow up and management of these patients If more study was used a significance reduction in the prevalence of diabetic nephropathy and its progression So we recommended to initiate and using determination, urine examination and renal function regularly in our practice to avoid and reduce the prevalence of diabetic nephropathy which lead to death
