Papillary carcinoma accounts for 85% of differentiated thyroid cancers.
We witness a yearly increased incidence and this is may be, in part, due to the increasing use of neck ultrasonography.
Certain histologic subtypes of papillary carcinoma have a worse prognosis related to vascular invasion, invasion into extrathyroidal tissues, extensive tumor necrosis and/or mitoses.
Adequate surgery is the most important treatment variable influencing prognosis, while radioactive iodine treatment, TSH suppression, and external beam irradiation each play adjunctive roles in at least some patients.
Numerous schemes have been developed in an effort to achieve more accurate risk factor stratification.
Each of the schemes allows accurate identification of the majority (70-85%) of patients at low risk of mortality (T1-3, M0 patients), allowing the follow-up and management of these patients to be less intensive than the higher-risk minority (T4 and M1 patients), who may benefit from a more aggressive management strategy.
Overall 5- and 20-year survival in the low-risk group was 100% and 99% respectively.
However, the survival in the high-risk group dropped to almost 72% and 57% respectively

Des cancers thyroïdiens bien différenciés, le carcinome papillaire représente 85% des cas.
On assiste à une augmentation de l’incidence annuelle, et ceci peut être en partie lié à l’augmentation de l’utilisation de l’échographie cervicale.
Certains types histologiques de carcinome papillaire ont un pronostic plus réservé du fait de l’invasion vasculaire, de l’invasion dans les tissus périthyroïdiens et la présence d’une nécrose tumorale extensive ou de mitoses.
Le traitement chirurgical adéquat est la variable la plus importante qui affecte le pronostic alors que le traitement par iode radioactif, la suppression de la TSH et la radiothérapie externe jouent chacun un rôle adjuvant dans certains cas.
Plusieurs schémas ont été développés pour essayer de mieux stratifier les facteurs de risque.
Chacun des schémas permet d’identifier la majorité (70-85%) des patients à faible risque de mortalité (T1-3, M0), et permet un suivi pour ces patients moins intensif que celui d’une minorité de patients à haut risque (T4 et M1), qui doivent bénéficier d’un traitement et d’un suivi plus agressifs.
La survie globale à 5 ans et à 20 ans dans le groupe à faible risque était de 100% et de 99% respectivement.
Cette même survie chez les patients à haut risque chutait à 72% et 57% respectivement
