Background : Many studies have shown that a considerable percentage of Asthenozoospermic men might harbor circulating antisperm antibodies in their sera and seminal plasma leading to immunological infertility.

Aim : To estimate the presence of seminal IgG antisperm antibody and the level of serum IgG antisperm antibody in asthenozoospermic patients and to compare the frequency of occurrence of ASABs in serum and seminal plasma taking in consideration their significance in relation to sperm parameters.

Patients and Methods : This study is a cross section-al analytical study which carried out 96 men attending the andrology outpatient clinic of Suez Canal university hospital complaining of primary infertility.

Each patient underwent semen analysis, seminal IgG antisperm anti body (MAR test) and serum IgG anti-sperm anti body done by ELISA.

Semen analyses were performed by computer aided sperm analysis (CASA) both dynamically and morphologically according to WHO2010 guidelines.

Seminal plasma IgG antisperm antibody : Sperm assessment was collected to carry out a direct mixed antiglobulin reaction (MAR) test.

Serum IgG antisperm Antibody : Sperm Antibody ELISA (Enzyme Linked ImmunoSorbent Assay) is a solid-phase sandwich enzyme-immunoassay for the quantitative determination of anti-spermatozoa antibodies in human serum.

Results : Our study showed that there is a significant relation between the presence of ASA in serum and semen and the seminal viscosity.

Seminal MAR test for ASA is highly sensitive than serum ASA ELISA detection, so it is a better diagnostic test.

There are about 20 % positive ASA patients among the isolated asthenozoospermic idiopathic primary infertile men.

Conclusion : The presence of ASA in serum or semen of men can affect the sperm motility leading to infertility.
