Microvascular complications of diabetes represents one of the most serious consequences of the disease, therefore the need for a biochemical marker with high predictive value for future microangiopathy is still required. Objective : To evaluate the measurement of serum levels type 111 procollagen peptide (P-Ul-P) as a non invasive marker for the development of diabetic microangiopathy. Research design and methods : A cross-sectional study on 62 diabetic patients(44 type 2 diabetes and 18 patients of type 1 diabetes) in addition to 20 non diabetic healthy subjects as a control group matched for age, sex and body mass index.

They were subjected for thorough clinical examination as well as fundoscopic examination by an ophthalmologist for detection of neuropathy and retinopathy, on the other hand their sera were examined for detection of serum procollagen type III, F.B.S glycosylated hemoglobin, lipid profiles alanine amino transferase (ALT), aspartate aminotransferase (AST), blood urea and serum creatinine, and urine for microalbuminuria. Results : There were significant differences between serum levels ofP-lll-P in diabetic patients and healthy control (P < 0.01), and among diabetics, between patients with microalbuminuria and those without (P < 0.01).

The correlation was highly significant between the duration of diabetes and the levels of serum P-II1-P (r = 0.79, P < 0.01).

Also the difference of serum P-11I-P levels in patients with retinopathy and in patients without retinopathy was significant (P < 0.01), while no differences in serum P-III-P in relation to F.B.S.

or glycosylated hemoglobin.

The differences in serum P-III-P levels in patients with and without polyneuropathy were highly significant (P < 0.01).

There were no significant differences of serum P-III-P in both types of diabetes in regards to the glycemic control.

Tlie sensitivity, specificity, positive predictive value, negative predictive value of serum P-III-P at selected cut-off value for detecting microalbuminuria were 96 %, 92 %, 88.8 % and 97.3 % > respectively, while it was 88 %, 69%, 76 % and83 % for retinopathy and 58%>, 86 %, 75 % and 74 % for detection of neuropathy.
