To evaluate the ability of intraumbilical oxytocin injection as treatment for retained placenta to reduce the incidence of manual removed and post-portum hacmonhage and to investigate factors in the patients or the infusion techniques which might determine the effectiveness of the method.

Methods Thirty women failed to deliver the placenta after 30 minutes of active management of the third stage of labour were included.

They received an intraumbilical injection of 20ml of normal saline if no evidence of placenta! reparation were received 20 I.U.

of oxytocin in 20ml of no!TOal saline.

The main outcome measures were the manual removed of the placenta and the postpartum hemorrhage.

An in vitro descriptive study was performed in freshly delivered placenta with sequential X ray films taken after injecting of varies volume of radiopaque solution intraumbilical.

Results the in vitro descriptive study showed that 20 ml was usually and 30 ml was always adequate to demonstrate placental capillaries in X ray.

Nine out of 30 patients responded positively to ١١١٧ injection of .؟aline alone and 10 out of 21 patients responded positively to the injection of 20 ml of oxytocin in normal saline intraumbilicaly by expelling the placenta within 30 minutes.

the rate of subsequent manual removal of the placenta and postpartum haemorrhage were significantly higher if the patient was multigraved and over 35 years of age.

There were no side effects and the method can be used safely as a first line for management of retained placenta.
