الملخص AR
يعتبر خلل الانتصاب الوظيفي (الضعف الجنسي) الناشئ عن خلل وعائي دموي واحد من أكثر الأسباب شيوعا و النوع الوريدي منه و الذي يعرف بالتسريب الوريدي هو عدم القدرة على احتجاز الدم في الأجسام الكهفية.
و قد أصبح للأشعة و الموجات فوق الصوتية دور هام في هذا المجال حيث يستخدم الآن جهاز الدوبلر الملون على نطاق واسع لفحص الأوعية الدموية و تحديد أسباب العنة الناتجة عن خلل في الأوعية الدموية سواء أكانت شريانية أو وريدية.
و يهدف هذا البحث إلى تقييم دور الدوبلر الملون في تشخيص حالات التسرب الوريدي مقارنة بفحص كافيرنوزوميتري و كافيرنوزوجرافي و قد شملت الدراسة ثلاثين مريض يعانون جميعا من العنة الناتجة عن خلل في الأوعية الدموية و قد أجريت الفحوصات الإكلينيكية الآتية : اختبار الحقن الموضعي و اختبار الدبلر الملون بالإضافة إلى اختبار كفاءة الأوردة عن طريق قياس الضغط داخل الأجسام الكهفية لبيان وجود التسريب الوريدي من عدمه.
و قد أظهرت هذه الدراسة أن الفحص بالدوبلر الملون هو فحص ممتاز كاختبار مسحي لتقييم حالات خلل الانتصاب الوظيفي الوريدي بطريقة سهلة و غير اجتياحية و في وقت قصير أما التشخيص الأكيد للذين يعانون من خلل وريدي فهو قياس الضغط داخل الجسم الكهفي هو ما يسمى بـ (كافيرنوزوميتري و كافيرنوزوجرافي).
الملخص EN
Today it is believed that vasculogenic ED is one of the most frequent causes of organic ED.
It may be (arteriogenic) or venoocclusive or both.
DIC is considered as the gold standard in the diagnosis of venoocclusive dysfunction.
Although CDUE of erectile dysfunction patients provides reliable information on arterial inflow, its role in veno-occlusive dysfunction assessment is still a matter of controversy.
Aim of the Work.
To evaluate CDUE in the diagnosis of veno-occlusive erectile dysfunction in comparison to Die.
Methods: work was conducted on thirty impotent patients, (24-74 years) with (mean 40.6 ± 11.6 years) and their mean duration of complains (25.6 months).
all our patients underwent complete evaluation, sexual, psychological and medical history, physical examination and routine laboratory tests, then several specific tests are done to evaluate penile vascular supply.
Results: the study showed that the sensitivity of CDUE (95.5 %), specificity (62.5 %), PPV (87.5 %) and NPV (83.3 %) in comparison to cavernosometry for the diagnosis of venous leakage.
So CDUE is a good negative test and Die proved to be the test of choice to exclude false positive cases of veno-occlusive ED, should be restricted to whom suspected to have venoocclusive dysfunction by CDUE and plane to be surgically managed.
Conclusion: Impotent patients should first, be assessed with OUT, which is simple and very informative.
Suspected vasculogenic case should be additionally evaluated by CDUE to dctect pure arterial insufficiency and suspect venoocclusive dysfunction which finally confirmed or excluded by Die, the gold standard in the diagnosis of venoocclusive ED although, It is invasive, expensive and time consuming
