Bilothorax is a rare condition, mostly associated with surgery involving the biliary system or trauma.

In this article a case of bilothorax secondary to liver surgery is reported, which recovered by pleural and abdominal drainage.

Bilothorax should be considered as a cause of respiratory detoriation in patients with recent biliary or hepatic surgery.
