Prevalence of peroneal to tibial anastomosis in a hospital sample

We describe two patients with peroneal-to-tibial anastomosis presenting as an initial positive phase n the compound muscle action potential (CMAP) response when recording at the extensor digitorum brevis (EDB) and stimulating the peroneal nerve at the fibular head.

No such initial positive phase was seen on the EDB CMAP response when stimulating the peroneal nerve at the anterolateral ankle.

Furthermore, the presence of an accessory deep peroneal nerve was excluded.

The frequency of the anastomosis is estimated to be about 10%, and is usually clinically significant only when there is significant motor axonal loss to the EDB.
