Objective: to assess the glucose tolerance, role ofinsaulin and plasma renin activity in the pathogenesis of polycystic ovary syndrome (PCOSJ, and to correlate their levels with age, body mass index (BMI) and waist to hip ratio.

Setting: In Gynaecology Outpatient Clinic ofAssiut University Hospital.

I Subjects: Forty women defined on clinical, hormonal and sonographic; background as PCOS andfourteen healthy age matched controls.

Interventions: Anthropomorphic measurements and laboratory determinations ! of serum FSH, LH, total testosterone glucose, and insulin (fasting and 2*hour after 75gm £ oral glucose load) and plasma renin activity (PRA).

3 Results: High percentages of impaired glucose tolerance (30%) and newly I discovered diabetes (5%) were detetcted in PCOS.

No significant difference was found \ in fasting serum glucose between patients and controls.

However, statistically | significant elevations were found in 2-hour glucose, fasting insulin, 2-hour insulin and | PRA in patients compared to controls (P<0.001, 0.01, 0.001, 0.001, respectively).In the I PCOS patients no significant differences were found between impaired glucose | tolerance (IGT) group and normal glucose tolerance (NGT) group as regards age, BMI 'j and waist to hip ratio.

No significant correlations were found between PRA and other 4 risk factors.

Conclusions: Impaired glucose tolerance and newly discovered diabetes are I common in PCOS.

Fasting serum glucose is a poor predictor of 2-hour glucose.

IGT is § common in obese as well as lean PCOS women, other risk factors are addedfactors.

% The role of PRA in PCOS requires further studies.

We recommendfollow up of PCOS f by oral glucose tolerance test for early discovery of IGT
