Leptin hormone, which is secreted from adipocytes, has a role in the regulation of appetite and energy expenditure.

Thyroid hormones influence metabolism and hence energy expenditure.

So this study aimed to evaluate the relationship between serum thyroid hormones, leptin levels, lipid profile and oxidative stress in thyroid dysfunction patients. Study was performed in the endocrinology clinic at the national diabetic center of Al-Mustansiryia University.

A total number of (82)women were used, (31women) hypothyroid patients, aged (18-52) year and (25women) hyperthyroid patients, aged (17-50) year and the other (26women) were healthy ( without thyroid gland dysfunction) , (14 obese, 12 non obese) aged (17-49) year.

Serum T3, T4, TSH, Leptin, lipid profiles , as well as the malondialdehyde (MDA) were examined. The results showed that the serum leptin level was significantly ( P < 0.05) higher in obese (46.53 ng/ml) than in nonobese (29.16 ng/ml) control women.

The results also showed a significant( P < 0.05) increase of leptin level in hypothyroid patients (80.81 ng/ml), and a significant( P < 0.05) decrease in hyperthyroid patients (14.50 ng/ml) compared with the of control group . The results revealed that there were a significant ( P < 0.05) increase in serum total cholesterol, triglycerides, high density lipoproteins cholesterol, very low density lipoproteins cholesterol and low density lipoproteins cholesterol in hypothyroid patients with high leptin level when compared with that of the control.

While there was significant (p<0.05) reduction in total cholesterol, High density lipoprotein cholesterol and low density lipoprotein cholesterol in hyperthyroid patients with low leptin level when compared with the of control group . b The results pointed out a significant (p<0.05) elevation in serum MDA level in hyperthyroid patients (3.12nmol/ml) and hypothyroid patients (3.06 nmol/ml) when compared with that of the control groups. In conclusion the results demonstrate that the disturbance in oxidation status in hypothyroid but not in hyperthyroid patients with abnormal lipid profile lead to the accumulation of lipids, hence to increase serum leptin concentration without activated (leptin resistance) and incidence of obesity related disease
