This study was conducted to determine the effect of erythropoietin in treatment of renal anaemia in a sample of Iraqi patients who were not on dialysis by evaluating a set of haematological and biochemical laboratory tests.

The study was performed during the period from June 2011 to January 2013, for this purpose 200 patients (100 males & 100 females), with chronic kidney disease and renal anaemia were studied in the department of nephrology (Dialysis center) in Al-Kindy Teaching Hospital, Baghdad / Iraq.

The age of the patients ranged from 45-75 years and all patients were diagnosed as having renal anaemia according to previous clinical reports, clinical examination and laboratory investigations.

Results were compared with 26 healthy control subjects, their age ranged between 45- 75 years.

All patients were given epoetin alfa (4000 IU) subcutaneously in the arms: once, twice or three times per weekly, depending on the clinical status of the patient.

patients were classified into two groups (group rhEPO responders and rhEPO non-responders) depending on the response to EPO therapy.

The two groups were divided into three sub-groups depending on the value of hemoglobin mild (Hb ≥ 10 g/dl), moderate (8 g/dL ≤ Hb < 10g/dL) and severe (Hb < 8 g/dL).

The achieved results were shown below: In case of the rhEPO responders - Haematological tests showed increase in Hb (g/dl) and Hct (%) levels during the period of EPO therapy, with decrease in ESR(mm/hr) levels .

VI - Biochemical tests showed increase in total protein (g/dl), albumin (g/dl), high density lipoprotein (mg/dl) and ferritin (mg/l) levels during the period of EPO therapy those were concomitant with decrease in creatinine (mg/dl), urea (mg/dl), potassium (meq/l), sodium (meq/l), cholesterol (mg/dl), triglyceride (mg/dl) and total iron binding capacity (mg/dl) levels during the period of EPO therapy.

In case of the rhEPO non-responders - Haematological tests showed decrease in Hb (g/dl) and Hct (%) levels during the period of EPO therapy, with increase in ESR(mm/hr) levels .

- Biochemical tests showed decrease in total protein (g/dl) and albumin (g/dl) levels during the period of EPO therapy, with increase in creatinine (mg/dl), urea (mg/dl), potassium (meq/l), sodium (meq/l) and ferritin (mg/l) levels.

There was no change in cholesterol (mg/dl), triglyceride (mg/dl), high density lipoprotein (mg/dl) and total iron binding capacity (mg/dl) levels.

Twenty four patients showed mild elevation in blood pressure (12%) particularly in the first 6 weeks .Two patients only developed discomfort at the site of injection due to local skin necrosis.

From the results it can be concluded that, the administred erythropoietin was improved many haematological and biochemical parameters levels in renal anaemia patients.
