This study has been carried out from April 2012 to march 2013.

It was included 500 sputum samples collected from patients who visitedspecialized chest and respiratory diseases center/TB reference lab in Baghdad.

Microscopically examination ofZeihl-Nelsen stain for the sputum samples have shown that 114 samples outof 500 samples were positive for TB.

114 samples were divided into two groups: new group 24 (21.1%) and old group 90 (78.9%).

The old group divided into four subgroups: defaulter 27 (30%), failure 22 (24.4%), relapse 21 (23.3%) and chronic 20 (22.2%).

All positive cases (114) were cultured on the solid media (Lowenstein-Jensen media).

Colonies were subjected for biochemical tests, 111 was identified as mycobacterium tuberculosis (97.4%) while three isolates were Non tuberculous mycobacteria (NTM) (2.6%).

Males showed the highest percentage of TB infection 72 (63.2%) as compared with females 42 (36.8%).

Drug susceptibility test for all isolates against streptomycin, isoniazid, rifampicin and ethambutol was carried out in liquid and solid medium.

The test showed that all isolates of new group (24) were sensitive to all antibiotics as well as (13) isolates out of oldgroup(90), while (77) patients of old group were resistant to at least two drugs (isoniazid and rifampicin) MDR-TB (multi-drug resistance cases).

The highest resistance percentage 18 (15.7%), 12 (10.5%) in males and females respectively was found in age group of 26-35 years, and no resistant was found in age group of 66-75 years.

Results of drug susceptibility test in both solid and liquid media using Bactec MGIT 960 showed 8(7%), 11 (9.6%) resistance for two drugs, respectively.

Resistant for three drugs wasin both solid and liquid media 30 (26.3%), 31 (27.2%), while four drugs resistance was both in solid and liquid media 39 (34.2%), 35 (30.7%).

37 (32.5%) of all drugs sensitive isolates in solid and liquid media.

Defaulter cases showed the highest resistance percentage to isoniazid and rifampicin 24 (31.1%), streptomycin 20 (25.9%), ethambutol 14 (18.2%)in both solid and liquid medium.Chronic cases showed the lowest resistance percentage to streptomycin 9 (11.7%), while relapse cases showed the lowest resistance to ethambutol 10 (13%), 8 (10.4%) by using solid and liquid media respectively.

The overall results of the present study indicate that Bactec MGIT 960 was most rapid and efficient system in detecting the resistance to anti-tuberculosis drugs and the results would be available in 7-14 days.

While, L.J medium needs 60 days for growth and another 30 days for sensitivity test estimation.

This fact may aid in controlling this disease and it's ultimately transmission in the society.
