Fifty-two childrens suspected of having visceral leishmaniasis (VL) and thirty healthy childrens as a control group collected from the Central Teaching Hospital of Pediatric in Baghdad were included in this study, during the period from December to March 2013 with age range between 5 months to 2 years (21 males and 31 females).

The duration of illness varied between 15 days to 4 months with majority of children being ill for more than one month.

Hepatomegaly and splenomegaly were seen in 22 (42.30%) and 30(57.69%) cases respectively.

Serum samples of each children were tested by using two type immunochromatographic dipstick assay (rKE-16,rK-39) to detect antibodies to leishmania donovani complex.

The results showed that 50 (96.15%) cases were confirmed to have infection by rKE16 dipstick test while, 46 (88.46%) cases were positive by rK39 dipstick test.

Non of the sample taken from healthy control sample showed reactivity in any of these tests.

Negative samples were submitted to bone marrow aspiration as a confirmatory test.

The results showed that all six negative patients were positively infected which giving the infection rate of 52/52 (100%).

Test of blood parameter (WBC,RBC,Hb) show significantly decrease (p<0.05) in patients with visceral leishmaniasis in comparison with control group.

On the other hand the mean concentration of WBC and RBC show significantly decrease (p<0.05) in patients with splenomegaly symptoms (2.24±0.29g/dl) and (2.26±0.34g/dl) respectively in comparison to patient with hepatomegaly symptoms (7.20±0.87g/dl),(3.15±0.23g/dl) respectively.

The result of Hb show non significant difference between patients with splenomegaly and hepatomegaly which more (7.20±0.74g/dl) and (7.53±0.91g/dl) respectively.

But show high significantly decrease (p<0.05) in comparison to control group (13.5±0.88g/dl).

Cellular (protective) and humoral (susptibility) immunity were evaluated by determination of IL-2 and IL-4 cytokines by using Enzyme-Linked Immunosorbent Assay (ELISA) kits.

A highly significant increase (p<0.05) in IL-2 was detected in subacute VL patients (152.53 ±16.4 pg/ml) in comparison to acute VL patients (19.63± 1.85 pg/ml) and control group (13.99 ±0.83 pg/ml).

On the other hand the IL-4 level increased significantly (p<0.05) in acute VL patients (169.72 ±19.3 pg/ml) in comparison to subacute VL patients (37.66± 2.67 pg/ml) and control group (20.06± 1.55 pg/ml).

Finally, the result of zinc levels in VL patients showed highly significant decreased (p<0.05) (47±2.58 mg/dl) in comparison to control group (82.8±4.64 mg/dl), results also showed a significant decrease (p<0.05) in zinc level of acute VL patients (40.2 ±2.16) in comparison to subacute VL patients (58.09± 3.08).
