Hypothyroidism is one of the most common functional disorders of the thyroid gland.

It has long been recognized that thyroid hormone have marked effects on glucose homeostasis.

The current study is carried out in the ‘specialized center of endocrinology and diabetes’ on (60) Iraqi hypothyroid patients for early detection of prediabetes and (30) randomly chosen individuals with normal thyroid function, the ages of the two groups were between (30-60) years and they were from the two sex.

The diagnosis of these patients has been performed under supervision of specialist physician in endocrinology.

The two groups were subjected to receive an oral glucose tolerance test (OGTT) and blood samples was collected for measurement of fasting blood glucose (FBG), insulin resistance (HOMA IR), glycated heamoglobine (HbA1c),high sensitive C-reactve protein(hs-CRP),thyroglobuline antibody (Tg ab), thyroid peroxidase antibody (TPO ab), and lipids profile (TC, TG, HDL-C, LDL-C, VLDL-C), in addition to measurement of body mass index( BMI).

The results of this study revealed that in hypothyroidism the majority of patients were females 54 (90%), while in men were 6 (10%).

According to criteria that submitted by the ADA it has been found that the majority group of hypothyroid patients (55%) was with prediabetes, (30%) was with diabetes and (15%) was non diabetes.

In this study a highly significant elevation (p<0.01) has been found in the level of insulin resistance in each of prediabetic and diabetic group, II Abstract moreover there were positive correlations (r>0.5) between thyroid stimulating hormone (TSH) and insulin resistance in these groups.

Highly significant increase (p<0.01) was found in the level of HbA1c, BMI in all of the study groups in addition, found positive correlations (r>0.35) between HbA1c and TSH, TSH and BMI in all study groups.

Regarding to the study of the correlation between BMI and FBG, BMI and HbA1c it has been found that there were significant correlations(r>0.35) between these parameters in all of the study groups.

Moreover, the level of the inflammatory marker hs-CRP was studied and it has been observed that there were highly significant elevation (p<0.01) in all groups compared to the control group, in addition to found a positive correlation (r>0.45) between hs-CRP and insulin resistance in these groups.

Estimation of the level of (Tg ab) in present study revealed that there was significant elevation (p<0.05) in diabetic group and prediabetic group, moreover there was found a positive correlation (r>0.35) between (Tg-ab) and insulin resistance, (Tg-ab) and HbA1c in these two groups.

A significant increase (p<0.05) in the level of (TPO ab) was observed in the diabetes group and there were a highly significant increase (p<0.01) in predaibetic and non diabetic group.

In addition to found a positive correlations (r>0.35) Between (TPO ab) and insulin resistance, (TPO ab) and HbA1c only in diabetic group and prediabetic group.

In the present study lipids profile have been tested in all groups and it was found that there were significant differences (p<0.05) in lipids profile (TC, TG, HDL-C, LDL-C,VLDL-C) when compared them between patients group and control group.

III Abstract Moreover, there was significant correlation (r>0.35) between TG and HbA1c, TG and insulin resistance, TG and BMI in diabetes, prediabetes, and non diabetes.

In this study it was concluded that hypothyroidism is a risk factor for insulin resistance, hyperlipidemia, and low grade inflammation; all of which can cause a state of pre-diabetes, and then to diabetes.
