Obesity is a major health crisis, and type 2 diabetes mellitus (T2DM) is one of its serious sequels.

It is associated with a state of chronic systemic inflammation that is a primary etiologic factor in development of insulin resistance in T2DM, mediated in large part by cytokines and adipokines products.

The present study aim to measure the expression of intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1) and other inflammatory markers and determine their role in type 2 diabetes mellitus.

Fifty Iraqi T2DM patients who were attended the National Diabetes Center for Treatment and Research at Al-Mustansiriya University during the period October 2012-March 2013 were recruited for this study.

For the purpose of comparison, 30 control subjects matched for age, gender and ethnic background (Iraqi) were also included.

The patients and controls were characterized in family history of diabetes.

Obesity measured by body mass index (BMI), waist-to-hip ratio(WHR), waist-toheight ratio (WHtR) and waist circumference (WC).

The patients were also assessed for duration of disease, fasting plasma glucose (FPG) and lipid profile [Total Cholesterol (TC), Triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL) and very low density lipoprotein (VLDL)].

The mean serum insulin (18.62 vs.

9.86 μIU/ml) and FPG (195.94 vs.

91.63mg/ml) levels were significantly higher (p< 0.001) in diabetic patients as compared to control group.

While a significant decrease(p< 0.001) was found in levels of HOMA- β cell % and HOMA- S% of diabetic patients in comparison to control group (59.62 vs.

114.22) and (40.5 vs.

83.9) respectively .

ii The mean serum VLDL (36.26 vs.

22.6 mg/dl) and TG levels (166.68 vs.

112.56 mg/dl) were significantly higher in T2DM patients as compared to control (p= 0.0002 and p= 0.0009 respectively).

Also the mean serum TC (172.08 vs.

159.66 mg/dl), and LDL (94.26 vs.

91.13 mg/dl) levels were higher in T2DM patients as compared to control but with no significant differences.

The mean serum HDL (45.08 vs.

46.8 mg/dl) level showed a decrease in T2DM patients as compared to control with no significant difference.

The mean serum IL-1β and hsCRP levels (29.55 vs.

19.87 pg/ml) and (23.98 vs.

7.05 mg/l) respectively were significantly (p< 0.0001) higher in T2DM patients as compared to control group.

The mean sICAM-1 serum level did not show a significant variation between patients and control group (57.62 vs.

60.05 ng/dl).

In contrast, serum VCAM-1 level (2017.3 vs.

1636.03 ng/ml) demonstrated a significant increase (p= 0.004) in T2DM patients as compared to control group.

Pearson correlation revealed a significant positive linear correlation between serum hsCRP and VCAM-1, LDL and FPG.

In contrast HOMA2-β% and VLDL showed a significant negative correlation with serum hsCRP.

IL1- β showed significant positive correlation with FPG and TG.

In conclusion, increased insulin resistance and decreased insulin sensitivity give rise to a hyperglycemic state that is a major risk factor for the development of diabetic complications.

Increased detectable levels of IL-1β, hsCRP, and VCAM-1 in T2DM patients compared with control suggests that an endothelial activation and the presence of inflammatory state associated with the type 2 diabetes which could be of importance for the accelerated atherosclerosis risk in newly diagnosed diabetic patients.
