Background Many autogenously graft materials have been used for closure of the drum perforation of the middle ear and ear canal.

More attention has been focused on the effectiveness of treatment modalities in relation to the costs.

Evaluation of treatment results in veconstmctive middle ear surgery with special regent to the type of the graft material is therefore of increasing importance. Aim: this study aimed at assessment of the functional results after myringaplasty and type I tympanoplasty with the use of different graft materials. Methods: this study included 250 patients out of 305 patients who presented with drum perforation at Damanhur Medical National Institute, ENT department. Myoringoplasty and type I lympanoplasty using fascia, perichondrium and cartilage.

Grafts were used in closing drum perforation.

Patients were tollowed up for 6 months.

On terms of post-operative closure of the air bone gap.

Fascial grafts were used in 110 patients, perichondria grafts in 50 patients and perichondnd cartilage graft in 90 patients Results: Post-operative functional results showed that the highest rate of perforation was recorded in cases when we used facial graft (20%) in comparison to perichondria graft (12%), and (7.7%) when we used cartilage graft.

The hearing results showed that the best hearing results were at frequencies 2 KHz for all the grafts used. Conclusions: The subjective improvement of hearing post operatively recorded 64% of the cases improved, while 36% of the cases denied any changes of their preoperative hearing.

Audio logical post-operative results in the three groups of patients using the three graft materials showed insignificant statistical differences in hearing improvement, although the best results were observed when we used perichondria and cartilage composite grafts for cases with subtotal and total perforation.
