Background : Heat shock proteins (HSPs) are a group of proteins present in all cells in all life forms.

There are many isoforms of HSPs, one of them is HSP32 which is known as heme- oxygenase-1 (HO-1) that catalyzes the metabolism of heme to biliverdin, iron and carbon monoxide.

Objective : to study the plasma level of HO- 1 and blood carboxyhemoglobin (CO-Hb) concentration in post- renal transplant recipients and their correlation to graft hemodynamics, function and survival.

Subjects &Methods : All subjects were subjected to proper history, clinical examination and laboratory investigations which included renal function tests, plasma HO-1, carboxyhemoglobin level.

Radiological study included ultrasound examination of the graft with measurement of resistive index (RI), plasticity index (PI), main artery renal blood flow and cross sectional area of the main renal artery (MRA).

Results : The present study showed that HO-1, CO-Hb% and serum creatinine were significantly increased in transplant recipients compared to controls.

At the same time, there was a statistically significant reduction of PHO-1, CO-Hb% and creatinine clearance in transplant recipients in rejection episodes than those without rejection.

At the same time, the present work had found a significant negative correlation between PHO-1, CO-Hb% on one hand and RI, PI on the other hand among renal transplant recipients.

Conclusion : PHO-1 and its byproduct CO-Hb have vital role in the control of the vascular tone of the graft vessels; and impairment of their production in graft rejection is associated with ischemic damage of the graft with subsequent deterioration of graft function and survival.
