Surgery has been the only effective treatment for morbid obesity, with long-term sustained weight loss and postoperative complete resolution or significant improvement in the obesity comorbidities.

Bariatric surgery is feasible and safe, when carried out in specialized medical institutions properly equipped to care for the obese in a multidisciplinary setting.

Laparoscopic sleeve gastrectomy (LSG) is a surgical procedure that is being increasingly performed on obese patients.

Gastric leak is one of the most dreaded complications following bariatric surgery as it can lead to significant morbidity and mortality.

We report a case of fistula after open sleeve gastrectomy in a patient who primarily diagnosed and managed in an incorrect way until he came to us where he received an appropriate treatment.
