Oxidative stress status and lipid profile in patients with thyroid gland disorders

Disorders of thyroid gland are amongst the most abundant endocrine diseases in the world.

Increase in reactive oxygen species induced by a disorder of thyroid hormones can lead to an oxidative stress.

So, the aims of this study are to investigate the antioxidant enzymes activity, malondialdehyd as indicator of lipid peroxidation and uric acid in both hyperthyroid and hypothyroid patients and to evaluate the relationship between thyroid dysfunctions with serum lipids and some trace elements levels (Zn, Cu and Se).

The total samples in this study are (205) divided as the following (47) hyperthyroid patients (22 males and 25 females) their age ranges between 15 – 61 years and (78) hypothyroid patients (21 males and 57 females), their age ranges between 16 – 64 years, in addition to (80) healthy subjects (26 males and 54 females), their age ranges between 15 – 60 years.

The statistical results of the current study showed a significant decrease in the thyroid hormones (thyroxine and triiodothyronine) levels, antioxidant enzymes activity (SOD , GPx , CAT and GST), HDL-C and serum trace elements (zinc and selenium) in hypothyroid patients compared to healthy subjects ( P < 0.001) , while serum copper level was insignificantly lower than that in the healthy subjects ( P > 0.05) .

A significant increases in thyroid stimulating hormone , MDA, uric acid and serum lipid (cholesterol , triglycerides, VLDL-C and LDL-C) levels in hypothyroid patients were noticed compared to healthy subject ( P < 0.001) .

Also, there were significant changes in serum thyroid hormones(T4 and T3 ), cholesterol , LDL-C , uric acid levels and activity of GPx enzyme in both sexes of hypothyroid patients , while there were no significant alterations in TSH, MDA , trace elements (Zn , Cu and Se) triglycerides, VLDL-C , HDL-C levels and activities of antioxidant enzymes (SOD , CAT and GST) .

III In addition , there were significant changes in triiodothyronine, uric acid and zinc levels in hypothyroid patients of different ages , while there were no significant alteration in other parameters with different age.

A significant decrease in the TSH level, activity of antioxidant enzymes (GPx, CAT and GST) and levels of trace elements (Zn and Se) were found in hyperthyroid patients compared to healthy subjects.

There were on the other hand, insignificant decrease in lipid profile and insignificant increase in copper level in hyperthyroid patients compared to healthy subjects.

There were also a significant increase in thyroid hormones (T4 and T3), MDA , uric acid levels and activity of SOD enzyme in hyperthyroid patients compared to healthy subjects .

The results showed significant changes in the activity of GPx enzyme , uric acid triglycerides and VLDL-C level in different sex , while there were no significant alteration in level of TSH, thyroid hormones (T4 and T3) , MDA , activity of antioxidant enzymes (SOD , CAT and GST), trace elements levels (Zn , Cu and Se) and levels of serum lipids (cholesterol , LDL-C and HDL-C)in different sexes in hyperthyroid patients .

It was also found that the age factor had no effect on TSH hormone, thyroid hormones (T3, T4), activity of antioxidant enzymes (SOD, GPx , CAT , GST) , MDA, uric acid, lipid profile and trace elements (Zn, Cu, Se) in the hyperthyroid patients.

The significant negative correlation of T4 and T3 hormones with TSH hormone were found in the hypothyroid patients.

Furthermore ,the level of TSH showed significant negative correlation with the level of CAT activity and significant positive correlation with the level of uric acid, cholesterol, VLDL-C, LDL-C and triglycerides in hypothyroid patients, while a significant positive correlation of T3 with T4 hormone and the level of MDA IV and significant negative correlation with cholesterol and LDL-C were found in the hypothyroid patients .

There was also a significant positive correlation of T4 level with MDA level and HDL-C and significant negative correlation with uric acid, cholesterol, triglycerides, VLDL-C and LDL-C in hypothyroid patients.

On the other hand, significant positive correlation of T4 hormone with T3, cholesterol and LDL-C levels were found in the hyperthyroid patients.

Our result indicated the relationship of zinc and copper to the activity of SOD enzyme.

We observed a significant positive correlation between the activity of SOD enzyme and the level of zinc in the hypothyroid and hyperthyroid patients.

The level of SOD activity also showed a significant positive correlation with the level of copper in hyperthyroid patients and insignificant positive correlation with the level of copper in hypothyroid patients , while insignificant negative correlation between the level of GPx activity and the level of selenium was found in the hyperthyroid and hypothyroid patients .

Finally, it is concluded that the thyroid dysfunction disease associated with oxidative stress is the consequence of both increased production of free radicals and reduced capacity of the antioxidant levels.
