Introduction : Extra-digestive manifestations are relatively common in chronic inflammatory bowel disease.
The etiopathogenesis remains obscure.
The most frequent manifestations are rheumatologic.
They may precede the onset of gastrointestinal symptoms by many months.
The goal of this paper is to review features and therapy of major extra-intestinal complications occurring in inflammatory bowel disease.
Patients and methods: Extra-digestive manifestations in 10 children diagnosed with inflammatory bowel disease between january 2005 and january 2016 in a pediatric department were retrospectively studied.
Results: Among the 10 patients, 4 aged 4 to 11 years develop at least one extra-digestive manifestation.
Its included osteo-articular complications in 3 patients: peripheral arthritis (1 case), osteopenia (1 case) and aseptic necrosis of the femoral head associated with osteoporosis (1 case).
Bronchectasis was reported in 2 cases.
Ophthalmologic exam revealed cataract in one case related to corticosteroids exposure.
One patient diagnosed with ulcerative colitis presented with 3 extra-digestive manifestations included hemolytic autoimmune anemia, sclerosing cholangitis and bronchectasis.
Conclusion: Extra-digestive manifestations show that inflammatory bowel disease can affect not only the bowel but also many other organs and can be considered to be multi-systemic disease.
They can be improved in parallel with the gastrointestinal symptoms or have independent disease course.
الملخص FRE
Introduction : Les manifestations extradigestives au cours des maladies inflammatoires chroniques de l’intestin sont relativement fréquentes pouvant précéder de plusieurs mois l’apparition des manifestations intestinales posant alors des problèmes diagnostiques.
Notre objectif est de préciser le profil clinique et thérapeutique des principales manifestations extradigestives des maladies inflammatoires chroniques de l’intestin chez l’enfant.
Patients et méthodes : il s’agit d’une étude rétrospective incluant tous les patients atteints de maladies inflammatoires chroniques de l’intestin et présentant des manifestations extradigestives suivis dans un service de pédiatrie générale durant une période de 11 ans (janvier 2005- janvier 2016).
Résultats : Dix patients atteints de maladies inflammatoires chroniques de l’intestin ont été suivis durant la période d’étude.
Les manifestations extradigestives ont été notées chez 4 enfants.
Il s’agit d’une atteinte ostéo-articulaire dans 3 cas : une arthrite du genou inaugurant une maladie de Crohn chez une fille de 8 ans, une ostéopénie dans 1 cas et l’association d’une ostéonécrose aseptique de la tête fémorale et d’une ostéoporose dans 1 cas.
Une dilatation des bronches a été retrouvée dans 2 cas.
L’examen ophtalmologique était pathologique dans 1 cas objectivant un glaucome.
Chez une patiente atteinte de rectocolite hémorragique, les manifestations extradigestives ont touché simultanément 3 organes occasionnant une anémie hémolytique auto-immune, une cholangite sclérosante et une dilatation des bronches.
Conclusion : Les manifestations extradigestives témoignent que les maladies inflammatoires chroniques de l’intestin ne sont pas purement intestinale mais une affection inflammatoire systémique pouvant atteindre de multiples organes.
Elles peuvent évoluer parallèlement à la maladie digestive ou pour leur propre compte.
