The fetal RHD genotyping from maternal plasma is a non-invasive innovative method based on the presence of cell-free fetal DNA in pregnant women plasma, therefore accessible simply through the collection of a maternal peripheral blood sample.

It allows the determination of fetal RH1 status in RH1sensitized RH :-1 women in which further surveillance is required.

It is also proposed for RH1 negative pregnant women to restrict the use of anti-D immunoglobulins only to those bearing an RH1positive fetus.

Several studies have pointed to the accuracy of fetal RHD genotyping with an excellent genotype/phenotype concordance.

الملخص FRE

Le génotypage RHD foetal sur plasma maternel est une méthode novatrice et non invasive dont le principe découle de la présence avérée d’ADN foetal à l’état libre dans le plasma maternel.

Les indications étaient limitées aux patientes alloimmunisées anti-RH1 pour adapter la surveillance et n’appliquer une surveillance spécialisée qu’aux foetus RH1, pour s’étendre ensuite aux patientes RH:-1 non immunisées afin de limiter l’injection d’immunoglobulines anti-RH1 aux seules patientes porteuses d’un foetus RH:1 et d’éviter les prophylaxies prénatales inutiles.

Plusieurs études dans la littérature ont démontré une excellente concordance entre le génotype RHD foetal prédit et le phénotype RH1 observé à la naissance.
