Protein creatinine index in random urine sample for early detection of diabetic nephropathy

26In a trial to find out a simple, cheap, easy method for early detection of diabetic nephropathy, protein I creatinine (PIC) ratio in random urine specimen, as well as 24-hour protein excretion compared to results obtained by albustix strips were studied in 10 subjects and 29 ambulent diabetics (IDD and NIDD), complicated and uncomplicated.

A striking high correlation was found between the protein creatinine index on random urine samples during normal day-time activities and total protein excretion on 24-hour urine determined by routine chemical methods where it was below 0.1 in healthy subject, the protein I creatinine index was found numerically equal to the total protein exaction when related to the creatinine excretion in gin.

albustix strips showed 61.5 % false negative results.

Finally a significant correlation was found between this ration and the duration of diabetes.

So, it is proposed that determination of the P / C ratio in single voided urine sample obtained during normal daytime activities can replace the 24-hour urine collection in clinical estimation of proteinuria and to supplement dipsticks where a ration more than 0.1 indicate diabetics who are further along the track and /or proceeding faster to nephropathy.
