Background: Type 2 diabetes mellitus (T2DM) is a multifactorial disease with a strong genetic component interacting with environmental factors.

Most of these genes have been linked to beta-cell dysfunction, impaired glucose homeostasis and insulin secretion.

Its prevalence rate in Gaza Strip is alarming.

Human genome studies revealed many T2DMassociated genetic polymorphisms in various populations.

Among the genes polymorphisms that were strongly associated with diabetes are (IGF2BP2 rs4402960), (IGF-2 rs680), (HHEX rs1111875), and (CDKN2A/B rs10811661).

Objective: To investigate the association between (IGF2BP2 rs4402960), (IGF-2 rs680), (HHEX rs1111875), and (CDKN2A/B rs10811661), genes polymorphisms and T2DM in Gaza Strip.

Methods: In this case-control study, 100 T2DM male patients and 100 control men were examined.

The two groups were genotyped for the four genes polymorphisms using restriction fragment length polymorphism-PCR (RFLP-PCR) technique.

Body mass index (BMI), glycated hemoglobin (HbA1c), insulin (C-peptide), total cholesterol, triglycerides, high density lipoprotein cholesterol (HDL-c), and low density lipoprotein cholesterol (LDLc) were measured for all the study participants.

The relation between the four genes polymorphisms, T2DM and the measured clinical parameters were statistically analyzed using appropriate tests.

Results: Among the tested polymorphisms, significant association were evident between IGF2BP2 "GG" genotype (P-value = 0.0001), and CDKN2A/B T-containing genotypes (Pvalue = 0.0032) and increased risk of T2DM.

The IGF2 "AA" genotype exerts significant effect on LDL-c level in patients (P value= 0.036), LDL-c levels were significantly higher in patients subjects with AA genotype as compared to patients with GG genotype.

Moreover, GG genotype exerted significant effect on HDL-c level in patients (P value=0.049), HDL-c levels were significantly higher in patient subjects harboring the GG genotype compared to patient with AA genotype.

The CDKN2A/B "CT" genotype exerts significant effect on Cpeptide level in patients (P value= 0.001), C-peptide level were significantly lower in patient subjects with CT genotype as compared to patients with CC genotype.

IGF2BP2 (rs4402960 T>C), and HHEX (rs4457053 A>G) genotypes did not reveal significant effects on the tested parameters.

Conclusion: IGF2BP2 "GG" genotype, and CDKN2A/B T allele genotypes are significantly associated with T2DM in the investigated population.

The IGF2 "AA" genotype exerts significant effect on LDL-c level in patients.

Moreover, GG genotype exerted significant effect on HDL-c level in patients.

CDKN2A/B "CT" C-peptide level were significantly lower in patient subjects with CT genotype as compared to patients with CC genotype.

The study recommends confirming the obtained results on a larger sample and examining the association of other genes polymorphisms with T2DM in Palestinian Population.
