Immunohematological diagnosis of autoimmune anemia is a clinic biological diagnosis.
the direct antiglobulin test (DAT) is the cornerstone of the etiological investigation of hemolytic anemia.
it allows the characterization of the hemolysis’s mechanism : immunological or non-immunological.
special attention should be paid to the interpretation of the results of TDA.
we will synthesize the immunohematological diagnosis to be undertaken if AHAI is suspected.
this biological exploration has a triple objective : to establish the diagnosis and the type of autoimmune conflict, to detect predictive factors of severity and to guarantee transfusion safety.
we will propose at the end an algorithm of immunohematological diagnosis of AHAI.
الملخص FRE
Le diagnostic des anémies hémolytiques auto-immunes est un diagnostic clinico-biologique.
Le test direct à l'antiglobuline (TDA) est la pierre angulaire de l'enquête étiologique d'une anémie hémolytique.
Il permet de caractériser le mécanisme de l'hémolyse: immunologique ou non immunologique.
Une attention particulière doit être apportée à l'interprétation des résultats du TDA.
Nous ferons la synthèse du diagnostic immunohématologique à entreprendre devant la suspicion d’une AHAI.
Cette exploration biologique répond à un triple objectif : établir le diagnostic et le type du conflit auto-immun, détecter les facteurs prédictifs de sévérité et garantir la sécurité transfusionnelle.
Nous proposerons à la fin un algorithme du diagnostic immunohématologique des AHAI.
