مرض الحمل العنقودي :- هو مرض شبه سرطاني يحدث في المشيمة حيث يكون هناك تورم و انتفاخ بخلايا المشيمة و يحتوي على عدة أشكال من الأورام التي تنشأ من خلايا المشيمة نتيجة لتلقيح معيب و لعدم وجود تسجيل لهذه الحالات في أي من مستشفيات دولة الكويت و لذا فقد قام قسم النساء و الولادة بمستشفى العدان بتسجيل و متابعة هذه الحالات في الفترة من 2001 و حتى 2007 و قد وجد أن نسبة الحمل العنقودي الكلية 2.74 % لكل ألف حالة ولادة موزعة كالتالي : 65 حالة (54 %) الحمل العنقودي الكامل و 41 حالة الحمل العنقودي جزئي و 7 حالات (5.8 %) الحمل العنقودي مستمر و 6 حالات (5 %) الحمل العنقودي السرطاني.
و هذا المرض علاجه بسيط جدا حتى في الحالات المتقدمة منه و ذلك عن طريق عقار الميثوتريكسيت شفاء تاما.
و لا يؤثر هذا المرض بعد الشفاء على خصوبة المرأة و ننصح بإجراء الفحص الهيستولوجي بجميع الأنسجة التي تستخلص من الرحم بعد عمليات الإجهاض و لابد من إنشاء مراكز متخصصة على المستوى القومي لمتابعة حالات الحمل العنقودي و تسجيلها و وضع خطط العلاج و تحديد معدل الانتشار و على هذا يمكن تحديد حجم المشكلة على مستوى كل دولة و تحديد الوسائل و المصادر لمكافحة هذا المرض شبه السرطاني الذي قد يقتل إذا لم يتم اكتشافه و علاجه في الوقت المناسب.
و الحمل الذي يحدث بعد حمل عنقودي يكون طبيعيا بدون زيادة في حالات الإجهاض أو الولادة المبكرة أو العيوب الخلقية أو أمراض حديثي الولادة.
Abstract EN
Gestational trophoblastic disease (GTD) encompasses a spectrum of neoplastic disorders that arise from placental trophoblastic tissue after abnormal fertilization.
As there was no specific registry program for GTD in Kuwait hospitals or at the national level ; we established a specialized clinic within the Department of Obstetrics and Gynecology at Al-Aden Hospital in Kuwait, during the study period From January 2000 to June 2007, a total of 43315 live births were recorded in our hospital.
The annual overall incidence of GTD with respect to live births was found to be 2.74 per 1000 live births.
There were 65 (54%), 41 (34.4%), 7 (5.8%), and 6 (5%) cases of non-persistent complete mole, partial mole, persistent complete mole, and choriocarcinoma respectively.
In conclusion, choriocarcinoma can be cured even though widely metastatic.
Because persistent trophoblastic disease may develop after any pregnancy it is recommended that all products of conception obtained after evacuation, should be examined histopathologically.
Successful management depends upon early, rapid, accurate diagnosis and institution of individualized therapeutic protocols.
