مرض تسمم الحمل يعتبر من أخطر المضاعفات التي تصاحب الحمل و الأجسام الشاردة قد تكون سببا في حدوث هذا المرض و الهدف من هذا البحث هو دراسة لتقييم نسبة الدهون و المالونديالديهيت و كذلك مضادات التأكسد مثل أنزيم الباروكسوناز و الأريل استيراس في السيدات الحوامل المصابات بمرض تسمم الحمل الشديد و مقارنتهن بالسيدات الحوامل اللاتي لا تعانين من ارتفاع بضغط الدم.
و قد أجريت هذه الدراسة على ستين سيدة تم اختيارهن في أعمار متقاربة و تم تقسيمهن إلى مجموعتين : المجموعة الأولى : و تشمل 40 سيدة من الحوامل المصابات بتسمم الحمل الشديد.
المجموعة الثانية : و تشمل 20 سيدة من الحوامل اللاتي لا تعانين من ارتفاع بضغط الدم كمجموعة ضابطة.
و قد أظهرت نتائج هذه الدراسة وجود زيادة ذات دلالة إحصائية في نسبة المالونديالدهيت و الكوليسترول و الدهون الثلاثية و الدهون ذات الكثافة المنخفضة و كذلك أنزيمات الكبد و البولينا و الكرياتينين في السيدات المصابات بتسمم الحمل الشديد مقارنتهن بالسيدات الحوامل اللاتي لا تعانين من ارتفاع ضغط الدم و كذلك وجود نقص ذو دلالة إحصائية في نسبة الدهون ذات الكثافة العالية و نشاط الباروكسوناز و الأريل استيراس في السيدات المصابات بتسمم الحمل الشديد عند مقارنتهم بالسيدات الحوامل اللاتي لا تعانين من ارتفاع ضغط الدم.
و يمكن الاستنتاج لأن زيادة أكسدة الدهون المصحوب بنقص في مضادات التأكسد مع الخلل في نسبة الدهون قد تكون سببا في حدوث تسمم الحمل.
Abstract EN
Pre-eclampsia is one of the most serious complications of pregnancy.
Free radicals have been implicated in the pathophysiology of pre-eclampsia.
The aim of this study was to evaluate lipid peroxidation, paraoxonase activity, arylesterase activity and serum lipid profiles in severe pre-eclamptic women.
The study included 40 Patients with severe pre-eclampsia in the third trimester with age range 26-31 years and 20 normotensive pregnant women also in the third trimester as a control group with age range 28-32 years.
Subjects and Methods: All studied individuals were submitted to full history taking, clinical examination, abdominal ultrasound, liver function tests {aspartate transaminase (AST), alanine transaminase (ALT)}, kidney function tests ( urea and creatinine), serum lipid profiles{ Cholesterol (Ch), Triglycerides (TG), Low Density Lipoprotein cholesterol (LDLc), High Density Lipoprotein cholesterol (HDLc)}, Malondialdehyde (MDA) as indicator of lipid peroxidation, paraoxonase (PON) and arylesterase activities.
Results: The study showed a significant increase of AST, ALT, urea, creatinine, Ch, LDLc and MDA in severe pre-eclamptic women when compared with normotensive pregnant women (P< 0.001), TG showed a significant increase in severe pre-eclamptic women when compared with normotensive pregnant women (P<0.05), while PON and arylesterase activities showed a significant decrease in Severe pre-eclamptic women when compared with normotensive pregnant women (P < 0.001), HDLc showed a significant decrease in Severe pre-eclamptic women when compared with normotensive pregnant women (P < 0.05).
A significant negative correlation was found between MDA and PON (r = -0.43, P< 0.05), while a positive correlation was found between HDLc and PON( r = 0.82, P < 0.001) and also a positive correlation was found between HDLc and arylesterase (r = 0.42, P < 0.05).
Conclusion: It could be concluded that excessive lipid peroxidation, decrease paraoxonase and arylesterase activities together with abnormal lipid profiles may have a role in the pathogenesis of pre-eclampsia.
