Laparoscopic ovarian drilling as anew method for treatment of luf syndrome

Abstract EN

Aim of the work : To determine whether laparoscopic ovarian drilling can be used in the treatment of luteinized unruptured follicle syndrome.

Patient and Method: This study was carried out at El-Hussien hospital on 40 unexplained infertile women diagnosed to having LUF syndrome from April 2003 to September 2005.

These women were selected from unexplained infertile women attended outpatient clinic.

Twenty women of them were receiving human chorionic gonadotrophin 10,000 IU intramuscular at follicular maturation(Group I).

Another twenty women were treated by laparoscopic unilateral ovarian drilling by electrocautery, 4 punctures 400 wt for 4 seconds(Group II).

FSH and LH was done before treatment to exclude PCO.

LUF syndrome was diagnosed ultrasonographically by failure to demonstrate a minimum 5 mm reduction in average follicular diameter with or without fluid in cul de sac within 48 hrs after LH surge was achieved.

Result: This study shows initial slow follicular growth in follicular phase of LUF cycle, accelerated follicular growth and rapid increase in follicular volume after LH surge.

There was 3 out of 20 women from group I were respond to 10,000 IU hCG by ovum release 15 % , only one of them got pregnant 5 % within 6 cycles.

There was 15 out of 20 women from group II respond to laparoscopic unilateral ovarian drilling by ovum release 75 % only 7 of them got pregnant 35% within 6 cycles.

Conclusion: There is a significant response to laparoscopic ovarian drilling in treatment of LUF syndrome but no other data was available.
