النزيف الرحمي الوظيفي يمثل مشكلة خطيرة لكثير من السيدات، و يعتبر من أهم أسباب استئصال الرحم في الوقت الحاضر.
إن كشط بطانة الرحم باستخدام الطاقة الكهربائية عن طريق منظار الرحم يعتبر طريقة ناجحة لعلاج النزيف الرحمي كبديل لاستئصال الرحم.
و قد أجريت هذه الدراسة في مستشفى باب الشعرية الجامعي للنساء و التوليد على ثلاثين سيدة تعاني من النزيف الرحمي الوظيفي و قد قسمن إلى مجموعتين كل مجموعة تتكون من 15 سيدة أجريت للأولى إزالة باطن الرحم عن طريق الكشط، و للثانية عن طريق التجلط.
و قد خضعت كل سيدة للكشف الطبي الدقيق يشمل التاريخ المرضي و الفحص الإكلينيكي و إجراء الفحوصات و الأبحاث اللازمة و كذلك تم عمل موجات فوق صوتية لتقدير حجم الرحم، و كذلك سمك بطانة الرحم بدقة.
و قد ثبت من البحث إن إزالة بطانة الرحم باستخدام الطاقة الكهربائية عن طريق منظار الرحم كعلاج النزيف الرحمي يعتبر بديلا جيدا لاستئصال الرحم.
و قد ثبت من البحث أيضا أن إزالة بطانة الرحم عن طريق التجلط تجري في وقت أقل من إزالة بطانة الرحم بطريقة الكشط و بالتالي أقل تعرضا للتخدير.
و لكن طريقة الكشط تتميز بأن نسبة انقطاع الطمث أكثر من طريقة التجلط و كذلك بإمكانية أخذ عينة للفحص الباثولوجي.
و قد خلص البحث إلى أنه على أطباء النساء ألا يقوموا بإجراء مثل هذه العملية قبل التدريب جيدا على استخدام المنظار الجراحي بالإضافة إلى الحاجة للمتابعة المتأنية و الطويلة لهذه العمليات لتقييم نتائجها و التوصل إلى أنسب الطرق للعلاج.
Abstract EN
Dysfunctional uterine bleeding is one of the most common causes of hysterectomies being performed today.
Aim of the Work: This study compares the effectiveness of hysteroscopic endometrial ablation using the resection technique and using coagulation technique as a surgical treatment for dysfunctional uterine bleeding.
Patients and Methods: 30 patients were recruited and divided into two equal groups: In the first group resection technique of endometrial ablation was done under general anaesthesia.
The second group, the coagulation technique of endometrial ablation using roller ball was done under general anaesthesia.
Conclusion: It seems that electrosurgical endometrial ablation is a viable and successful alternative to hysterectomy for the treatment of perimenopausal dysfunctional uterine bleeding and not responding to hormonal therapy or curettage operation.
It can be performed safely and the complications rates compares favorably with hysterectomy.
However, resection technique is more effective as it is followed by a high incidence of amenorrhoea, it provides an operative specimen for histopathological examination and it is more suitable for thick endometrium as the depth of resection can be controlled.
