هدف الدراسة : دراسة العلاقة بين عوامل متلازمة الأيض (metabolic syndrome) و مرض قصور الدورة الدموية التاجية (IHD) في كلا الجنسين.
طرق الدراسة : أجريت الدراسة على 1387 من المرضى في ثلاث مستشفيات مختلفة.
تم إجراء الفحص السريري مع التحاليل الخاصة لبيان مدى الإصابة بعامل الأيض المركزي (و يشمل عامل كتلة الجسم، ارتفاع نسبة دهون الثلاثية (ترايكلسرايد) (Triglycerides) مع هبوط نسبة الدهون العالية الكثافة (HDL) بالإضافة إلى عامل السكر و عامل الضغط.
يعرف وجود متلازمة الأيض على أساس توفر ثلاثة عوامل أو أكثر من العوامل الخمسة.
النتائج: أثبتت النتائج أن هناك علاقة قوية بين مرض قصور الدورة الدموية التاجية و عوامل المتلازمة الأيضية التي تشمل السمنة، ارتفاع ضغط الدم، ارتفاع نسبة السكر في الدم، ارتفاع نسبة دهون الثلاثية (ترايكلسيرايد) و هبوط نسبة الدهون العالية الكثافة.
وجدت المتلازمة الأيضية في 48% من مرضى القلب المصابين بقصور الدورة الدموية التاجية، مع غلبة واضحة للإناث 60%.
كما تبين أن ارتفاع ضغط الدم كان من أكثر العوامل شيوعا 82% في مرضى القلب لكلا الجنسين، بينما مثلت السمنة 57%، السكري 20% و كلاهما أكثر حدوثا في الإناث.
كذلك وجدت الدهون الثلاثية عالية في 35% و الدهون العالية الكثافة واطئة في 23%.
أثبتت النتائج صحة النظرية التي تذهب إلى أن المتلازمة الأيضية تظهر تأثيرا لها من خلال مختلف عوامل الخطورة بواسطة طرق و ميكانيكية مختلفة.
الخلاصة: المتلازمة الأيضية هي شائعة بين مرضى قصور الدورة الدموية التاجية و له علاقة بعوامل الخطورة القابلة للتغيير.
Abstract EN
Objectives : to study the association between metabolic syndrome components and ischemic heart disease (IHD) in Both sexes.
Patients & Methods: A total of 1387of adult in-patients attending three different hospitals were studied.
Clinical examination and specific investigations to clarify the incidence of a central metabolic factor (body mass index BMI, high serum triglycerides TG and low high density lipoprotein HDL), a glucose factor and a blood pressure factor were carried out.
The presence of metabolic syndrome was defined as the presence of three or more risk factors.
Results : there were significant positive association between IHD defined by resting electrocardiogram (ECG) criteria and the components of metabolic syndrome including obesity, elevated blood pressure, fasting and post challenge hyperglycemia, high TG and an inverse significant association with HDL cholesterol.
Metabolic syndrome was present in 48% of studied IHD patients, with clear predominance of females 60%.
Hypertension was the commonest risk factor in both males and females (82%), obesity 57%, diabetes 20%; both more common in females.
High triglycerides present in 35%, while low HDL in 23%.
These results support the thesis that the metabolic syndrome exerts effects through different risk factors by different mechanisms.
Conclusions : the metabolic syndrome is common among ischemic heart disease patients and is related to modifiable risk factors.
