Background : In prolonged diabetic state hyperglycemia leads to formation of the AGEs that react with its receptors (RAGEs) in the periodontal tissues leads to periodontal tissue destruction.

The sRAGE is present normally in the body and acts as a competitive inhibitor for RAGE preventing its interaction with the AGEs and decreasing the tissue destruction.

Aim : To test the effect of periodontal therapy on the plasma level of the sRAGE in type 2 Diabetes Mellitus (non insulin dependent diabetes).

Method : 60 subjects affected by chronic periodontitis were grouped into Three groups ; Group I healthy control subjects (n = 20), Group II well controlled diabetic subjects affected by type 2 DM (n = 20) and Group III poorly controlled diabetic subjects (n = 20).

The subjects were examined by the clinical periodontal parameters (GI, PLI, PD and CAL) every patient was subjected to non surgical periodontal therapy combined with 100mg doxycyclin twice daily.

Blood samples were collected at baseline and after one month to be used for determination of the level of the plasma sRAGE before and after periodontal therapy by the use of ELISA kit.

Results : At baseline the sRAGE was detected in 90 % of the non diabetic subjects with mean levels740.87 ± 60.13 pg / ml of blood and in 40 % of the controlled diabetic subjects with mean levels 178.42 ± 38.22 pg / ml of blood and in 15 % of the poorly controlled diabetic patients with mean levels 82.89 ± 26.56 pg / ml of blood.

After periodontal therapy the plasma levels of sRAGE was increased in the studied three patients’ groups.

Conclusions : Serum levels of sRAGE was inversely correlated with the degree of diabetic state and inversely correlated with the scores and measures of the clinical periodontal parameters.
