Objective : To determine the ratio of total cholesterol to high-density lipoprotein cholesterol in coronary heart disease and myocardial infarction patients.

Methods: Five hundred and seventy nine (579) subjects were studied.

Their age ranged between 25 - 72 years.

Patients with coronary heart disease and myocardial infarction were selected from Queen Alia Heart Institute and Queen Alia Military Hospital clinics.

The control group was selected from house staff.

The enzymatic method was used and analysis was done within 24 hours using the Hitachi 911, 704 auto analyzers.

Results : The mean value of cholesterol in males and in females with coronary heart disease and myocardial infarction patients was higher by 25 % than that of the control group.

Total cholesterol to high-density lipoprotein cholesterol ratio in female patients with coronary heart disease was higher than that in males, while patients with myocardial infarction, had approximately an equal ratio.

The percentage value of cholesterol : High-density lipoprotein ratio (> 5) in the myocardial infarction group that was less than that in the coronary heart disease group.

Myocardial infarction patients have coronary heart disease risk of > 15 % according to the Sheffield table.

Conclusion : High-density lipoprotein cholesterol can be helpful to the clinician among other risk factors.

The total cholesterol to high-density lipoprotein cholesterol ratio may be of value to identify people at risk.

The value of 5-6 plus coronary heart disease risk > 15 % indicate a high risk in coronary heart disease and myocardial infarction patients.
