Orbital rhabdomyosarcoma usually presents with unilateral proptosis, dislocation of the globe and impairment of motility in childhood.

We hereby report a rare case of orbital rhabdomyosarcoma in a 13-year-old boy who presented with an epibulbar inflamed nodule.

The atypical presentation and the unpredictable behavior mandated documentation of the case.
