هدف البحث : تقييم النمو الجسمي و النضج البلوغي عند مجموعة من الأطفال المصابين بالنمط الأول للداء السكري بالمقارنة مع مجموعة شاهد. طرق البحث : تم في هذه الدراسة (دراسة الحالات و الشواهد) تقييم النمو الجنسي و معدل النضج الجنسي لدى 40 طفلا (26 ذكور، 14 إناث) ضمن الفئة العمرية 8 - 18 سنة من المصابين بالداء السكري من النمط الأول و إجراء مقارنة مع حالة 40 طفلا آخرين سويين صحيا كمجموعة شاهد موافقة من حيث العمر و الجنس.
تم تقييم الطول، الوزن، مشعر كتلة الجسم BMI و نسبة النضج الجنسي SMR بالمقارنة مع المجموعة العمرية الموافقة. النتائج : لوحظ أن الطول لدى 10 مرضى (بنسبة 25%) كان أقل من مستوى الشريحة المئوية الخامسة، بينما توضع 29 مريضا (بنسبة 72.5%) بين الشريحة المئوية الخامسة و الخامسة و السبعين، و مريض واحد (2.5%) أعلى من الشريحة المئوية الخامسة و السبعين و ذلك بالمقارنة مع توضع مجموعة الشاهد : مريض واحد (2.5%), 23 مريضا (57.5%) و 16 مريضا (40%) على الترتيب.
أما بالنسبة للوزن فقد لوحظ أن الوزن لدى 11 مريضا (27.5%) كان أقل من مستوى الشريحة المئوية الخامسة، بينما كان بين الشريحة المئوية الخامسة و الخامسة و السبعين لدى 26 مريضا (65%)، و أعلى من الشريحة الخامسة و السبعين لدى 3 مرضى (7.5%) و ذلك بالمقارنة مع توضعات نتائج مجموعة الشاهد في المجموعات الثلاث : مريض واحد (2.5%), 25 مريضا (62.5%) و 14 مريضا (35%) على الترتيب.
بلغ متوسط مشعر كتلة الجسم BMI لدى مجموعة المرضى 19.52 ± 3.48 بينما بلغ 21.41 ± 4.80 في مجموعة الشاهد.
لوحظ أن متوسط العمر عند بدء الطمث 13.30 ± 1.15 سنة لدى مجموعة المرضى مقارنة ب 12.69 ± 0.85 سنة لدى مجموعة الشاهد.
كانت نسبة النضج الجنسي طبيعية عند 24 مريضا من مرضى السكري (60%) بالمقارنة مع 35 مريضا (87.5%) في مجموعة الشاهد، بينما لوحظ تأخر في النضج الجنسي عند 16 مريضا من مرضى الداء السكري (40%) بالمقارنة مع 5 مرضى (12.5%) لدى مجموعة الشاهد. الاستنتاجات : يترافق النمط الأول للداء السكري عند الأطفال مع تأخر في النمو الجسدي و النضج الجنسي.
Abstract EN
Objective : The aims of this study were to assess the physical growth and pubertal maturation in a group of children of type 1 diabetes mellitus compared with controls. Methods: In this case-control study, physical growth assessment and sexual maturity rating of 40 patients aged 8-18 years (26 males, 14 females) with established type 1 diabetes were compared with those of 40 healthy controls of the same age and gender.
Assessment of height, weight, Body Mass Index (BMI) and Sex Maturity Rating (SMR) were studied in comparison to a matched age group. Results: The height of 10 patients (25%) was less than 5th percentile, twenty nine patients (72.5%) were between 5-75th percentile and one (2.5%) above 75th percentile in comparison with the results of the controls: one (2.5%), 23(57.5%) and 16 (40%) respectively.
The weight of 11 patients (27.5%) was less than 5th percentile, twenty six patients (65%) were between 5-75th percentile and three patients (7.5%) above 75th percentile in comparison with the results of the controls: one (2.5%), 25 (62.5%) and 14 (35%) respectively.
The mean BMI was 19.52±3.48 in patients whereas in the control group it was 21.41±4.80.
The mean age of starting menarche in diabetic patients was 13.30±1.15 years in comparison to 12.69±0.85 years of controls.
Twenty four diabetic patients (60%) had normal SMR compared to 35(87.5%) of controls, and sixteen patients (40%) had delayed SMR compared to 5(12.5%) of controls. Conclusions: It is concluded that diabetic children (type 1) are associated with impairment of physical growth and delayed sexual maturation.
