هدف البحث : دراسة دور المفطورات الرئوية في حالات ذات الرئة عند الأطفال في محافظة بابل في العراق.
طرق البحث : شملت الدراسة 83 طفلا من الجنسين، أعمارهم بين شهرين و 14 سنة، تم قبولهم في مشفى الأطفال و الأمومة كحالات ذات رئة.
تم إجراء الفحوصات المصلية من خلال المقايسة المناعية الأنزيمية EIA للأضداد IgM النوعية للمفطورات الرئوية، كما تم جمع البيانات السريرية و السكانية المتعلقة بكل حالة.
النتائج : لوحظت إيجابية الأضداد المصلية النوعية للمفطورات الرئوية عند 12 من أصل 83 مريضا (بنسبة 14.45 %).
لوحظت زيادة في انتشار الإصابة بتقدم العمر لتصل حتى 66.6 % عند الأطفال بسن 5 ≤ (0.05>p).
لم تلاحظ فروقات هامة إحصائيا في توزع الحالات بالنسبة للجنس، مكان الإقامة، مع وجود فروقات هامة إحصائيا في موضوع بدء تظاهر الحالة.
لا تتميز الفحوصات المخبرية الروتينية و الموجودات الشعاعية بنوعية كافية لوضع التشخيص المؤكد لذات الرئة بالمفطورات الرئوية، و لهذا فإن المعالجة التجريبية تستخدم عادة إلا في حالات وضع التشخيص المؤكد بالاستقصاءات النوعية.
الاستنتاجات : يلاحظ توارد عال للإنتانات بالمفطورات الرئوية عند الأطفال و خاصة بسن الخامسة فما فوق.
يجب العمل على زيادة توفر الفحوصات المخبرية النوعية اللازمة لتشخيص هذه الحالات (تحري الأضداد النوعية IgM) لتجنب المعالجات التجريبية من جهة و الوقاية من تطور الاختلاطات من جهة أخرى.
Abstract EN
Objective: To investigate the role of mycoplasma pneumoniae in pediatric pneumonia in Babylon-Iraq.
Methods: Eighty three children of both sexs aged between 2 months and 14 years were admitted to Babylon hospital for maternity and children as cases of pneumonia.
They were examined serologically by enzyme immunoassays (EIAs) for specific IgM for mycoplasma pneumoniae.
Demographic and clinical data were obtained.
Results: Twelve patients out of 83 (14.45%) were positive for mycoplasma pneumoniae.
The prevalence increased with age to reach 66.6% for those >5 years old (p<0.05).
No significant statistic findings regarding distribution of cases for sex, residence, but significant for onset.
Routine laboratory tests and radiological features are not specific enough to give an accurate diagnosis of these infections, so empiric regimens might be used unless diagnosis was done by rapid diagnostic procedure.
Conclusions: There is a high frequency of mycoplasma pneumoniae infection in children more frequent in those > 5 years old.
Thus specialized rapid laboratory testing for those agents should be more widely used as IgM serology diagnostic tests to avoid complicated course and empiric regimen.
