هدف البحث: تحديد العوامل المسببة للاتهاب الأذن الوسطى عند الأطفال حتى عمر 15 سنة، و تحديد الصادات الحيوية المناسبة للمعالجة و مقارنتها مع الصادات المعطاة تجريبيا من قبل أطباء الأطفال في اليمن.
طرق البحث: تم الحصول على نتائج الزرع و التحسس الجرثومي ل 429 عينة من مسحات الأذن الوسطى مأخوذة من أطفال أعمارهم دون 15 سنة يعانون من التهاب أذن وسطي مقاوم للأدوية المعطاة.
أخذت نتائج الزروع من المخبر الجرثومي الرئيسي في مدينة صنعاء خلال الفترة من حزيران 2006 و حتى الشهر نفسه من عام 2007.
من جهة أخرى طلب من عدد من أطباء الأطفال (50 طبيبا) تسجيل خمسة من الصادات الحيوية التي يعتمدونها في معالجة حالات التهاب الأذن الوسطى و ذلك لمقارنة المعالجة التجريبية مع نتائج الزرع و التحسس الجرثومي.
النتائج : شمل البحث 429 طفلا، 145 منهم ( 33.8%) بعمر سنتين فما دون، 101 (23.5 %) أعمارهم بين 5-3 سنوات، 183 (42.7 %) أعمارهم 6 سنوات فما فوق.
شكل الذكور نسبة 62 % من المرضى بينما شكلت الإناث 38 %.
شملت الجراثيم الأكثر تواردا كسبب لالتهاب الأذن الوسطى ما يلي على الترتيب : العنقوديات المذهبة (41.7 %)، الزوائف الزنجارية (أو العصيات الزرق) (17.7 %)، ذراري المتقلبات (11.2%) و المستدميات النزلية (7.7 %).
أما بالنسبة للصادات فقد بلغت حساسية العناصر الممرضة لدواء ceftriaxone (77.7 %)، يليه cefotaxime (72 %)، cefuroxime (69.1 %)، amoxil / clavulanic (57.3 %).
لوحظ أن الصادات الأكثر استخداما من قبل أطباء الأطفال كمعالجة تجريبية هو amoxil/clavulanic (92 %).
لوحظ أن أطباء الأطفال الممارسين اعتادوا في البداية وصف دواء amoxil/clavulanic، كما أن نسبة هامة منهم يصفون amoxil و الذي لوحظ أنه عديم الفعالية.
الاستنتاجات : تمثل جراثيم العنقوديات المذهبة، الزوائف الزنجارية و ذراري المتقلبات العناصر الممرضة الأكثر تورطا في حالات التهاب الأذن الوسطى عند الأطفال في اليمن.
لوحظ أن العناصر الممرضة المسببة لالتهاب الأذن الوسطى حساسة لكل من ceftriaxone، cefotaxime و amoxil/clavulanic على الترتيب.
Abstract EN
Objective: To identify the causative agents of otitis media of children aged up to 15 years, and determine the proper antibiotics and compare them with those empirically prescribed by Yemeni pediatricians.
Methods: A total of 429 bacteriology results of middle ear swabs, agent microorganisms, and their susceptibility to antibiotics of children aged up to 15 years old with otitis media resistant to prescribed antibiotics were collected from three main microbiology lab in Sana’a city during the period from June 2006 till June 2007.
Fifty pediatricians were asked to list their 5 antibiotics of choice in treatment of otitis media to compare empirical prescription with microbiology results.
Results: A total of 429 children were enrolled in this study, 145 (33.8% ) of them aged 2 year or less, 101 between 3-5 years (23.5%), while 183 aged 6 years old and above (42.7%).
Males were 266 (62.0%) while 163 (38.0%) were females.
The most common pathogens were Staphylococcus aureus (41.7%), Pseudomonas aeruginosa (17.7%), Proteus spp (11.2%) and Hemophilus influenza (7.7%).
Concerning antibiotics, pathogens were more sensitive to ceftriaxone (77.7%) followed by cefotaxime (72.0%), cefuroxime (69.1%), amoxil\clavulanic (57.3%).
Most common empirically prescribed antibiotics were amoxil/clavulanic (92%).
Practicing pediatricians used to prescribe amoxil/clavulanic first and a considerable proportion are prescribing amoxil which is not effective.
Conclusions: Staphylococcus aureus, Pseudomonas aeruginosa and Proteus spp.
are the most common pathogens associated with otitis media in Yemeni children.
Otitis media pathogens are sensitive to ceftriaxone, cefotaxime and amoxil/clavulanic respectively.
