هدف البحث : إعطاء فكرة حول أمراض الغدة الدرقية في العراق، و التغير الحاصل في سلوك هذه الأمراض و توجهات الجراحين نحو جراحة أكثر جذرية في تدبيرها.
طرق البحث : أجريت مجموعة من العمليات على 224 مريضا قام بها جراح واحد خلال فترة 15 عاما.
تم تقسيم المرضى إلى مجموعتين : الأولى ضمن الفترة بين كانون الثاني 1990 و حتى حزيران 2000، و الثانية ضمن الفترة بين حزيران 2000 و حتى حزيران 2005.
النتائج : لوحظ أن نسبة الإناث إلى الذكور لدى مرضى البحث هي 1:3.3.
أكثر التظاهرات شيوعا في المجموعتين هي السلعة العقيدية المتعددة GNM بنسبة 76.8 %.
سجل وجود سلعة سمية في 24 % من الحالات مع زيادة لدى المجموعة الثانية.
لوحظ وجود تأثير هام للشدة النفسية (الكرب sress) في زيادة حدوث الانسمام الدرقي و ذلك عند 50 % من مرضى الانسمام.
شكلت الأورام الدرقية الخبيثة 8.5 % من أمراض الغدة الملاحظة، و كانت معظمها من نوع السرطانة الحليمية.
لوحظت زيادة في حدوث أورام الغدة الدرقية الخبيثة لدى المجموعة الثانية من المرضى (2.65 % في المجموعة الأولى مقابل 13.5 % في المجموعة الثانية).
تبين وجود توجه لجراحة أكثر جذرية لدى المجموعة الثانية من المرضى.
الاستنتاجات : توجد زيادة في خباثات الغدة الدرقية و الانسمام الدرقي في بلدنا العراق، كما أن عوامل الشدة النفسية ربما تلعب دورا في هذه الزيادة.
و نتيجة للتغيرات السابقة، يوجد تغير في التوجه العام لدى الجراحين نحو إجراء أكثر جذرية في التدبير.
Abstract EN
Objective: to give an idea about thyroid diseases in Iraq and the change in the behavior of the diseases and the trends of the surgeons toward more radical surgery. Methods: Two hundred and twenty four patients were operated upon by the same surgeon during 15 years period.
Those patients were divided into two groups (group I from June 1990 up to June 2000) and (group II from June 2000-June 2005). Results: Female to male ratio was 3.3:1.
The most common presentation among both groups was Multinodular goiter (MNG) 76.8%.
Toxic goiter was recorded in 24% of patients with increase among group II patients.
A significant effect of stress on the increasing incidence of thyrotoxicosis was elicited in more than 50% of patients with thyrotoxicosis.
Malignant thyroid tumors constituted 8.5% of the diseases and they were mostly papillary carcinoma.
There was an increase in the incidence of malignant thyroid tumors among group II patients (2.65% in group I versus 13.5% in group II).
The trend toward more radical surgery was evident among group II patients. Conclusions: thyroid malignancy and thyrotoxicosis are increasing in our country and stress factor might play a role in this rise.
Due to the previous changes, the surgeon attitude had changed toward more radical surgery.
