Abstract AR
هدف البحث : يمثل التصوير الومضاني للأقنية الدمعية الأنفية طريقة آمنة و سهلة الإجراء و لا تحتاج لأي إجراء تداخلي, و هي تستخدم في تقييم المرضى الذين يعانون من الدماع.
يهدف هذا البحث إلى تقييم إمكانية و فائدة تصنيف الإنسداد الوظيفي للأقنية الدعية الأنفية بحسب مستوى الانسداد عن طريق التصوير الومضاني للأقنية الدمعية الأنفية.
طرق البحث : ضمت الدراسة ثلاثين مريضا بموجودات مرضية بالتصوير الومضاني للأقنية الدمعية الأنفية يعانون من دماع أحادي أو ثنائي الجانب دون وجود زيادة في إفراز الدمع أو انسداد لدى إجراء فحص الحقن.
تم تقييم كل الصور من قبل طبيبين مختصين في الطب النووي، و اعتبار رأي طبيب ثالث عند عدم التطابق في الآراء.
النتائج : قسم المرضى تبعا للنتائج إلى ثلاث فئات حسب مستوى الانسداد ك الفئة A (انسداد الأقنية الدمعية الأنفية الوظيفي ما قبل كيس الدمع) حدث فيها تأخر في الإطراح مع عدم امتلاء الكيس الدمعي و ضمت 9 أعين من أصل 44 عينا.
الفئة B (انسداد الأقنية الدمعية الأنفية الوظيفي ما قبل الأقنية الدمعية الأنفية) لوحظ فيها امتلاء في كيس الدمع جون وصول أية فعاليات ومضانية إلى القناة الأنفية الدمعية و ضمت 19 عينا من أصل 44 عينا، الفئة C (انسداد الأقنية الدمعية الأنفية الوظيفي ضمن الأقنية الدمعية الأنفية) لوحظ فيها فعالية ومضانية في القناة الأنفية الدمعية دون وصول هذه الفعالية إلى الأنف و ضمت 16 عينا من أصل 44 عينا.
الاستنتاجات : يمثل التصوير الومضاني للأقنية الدمعية إجراء سهلا و دقيقا في عملية تشخيص و تصنيف حالات الانسداد الوظيفي للأقنية الدمعية الأنفية بحسب مستوى الانسداد إلى (انسداد ما قبل كيس الدمع، ما قبل الأقنية الدمعية الأنفية، و انسداد ضمن الأقنية الدمعية الأنفية)، الأمر الذي يساعد بشكل كبير في إجراء التدبير السريري المناسب للحالة.
Abstract EN
Objective : Dacryoscintigraphy is a non-invasive, safe and easy method to evaluate patients complaining of epiphora.
We conducted this study to assess the value of dacryoscintigraphy in classifying functional nasolacrimal duct obstruction according to level of obstruction.
Methods : This study included thirty patients with abnormal findings in dacryoscintigraphy, complaining of unilateral or bilateral epiphora, with no tear overproduction and easy passage on syringing.
All images were interpreted blindly by two nuclear medicine specialists, third opinion was considered in case of disagreement.
Results : There were 9 out of 44 eyes with delayed excretion and no filling of the lacrimal sac ; this group was classified as functional nasolacrimal duct obstruction at the presacal level (Class A).
There were 19 out of 44 eyes with activity filling the lacrimal sac but no radiotracer reaching the nasolacrimal duct, this group was classified as functional nasolacrimal duct obstruction at preductal level (Class B).
Eyes with radiotracer reaching the nasolacrimal ducts but no activity reaching the nose were 16 out of 44 eyes ; this group was classified as functional nasolacrimal duct obstruction at intraductal level (class C).
Conclusions : Functional nasolacrimal duct obstruction is easily diagnosed with dacryo- scintigraphy.
This scan can accurately classify level of obstruction into presacal level, preductal level and intraductal level, which may have great impact on clinical management.
