هدف الدراسة : تهدف الدراسة إلى وصف التظاهرات و التدبير و النتائج للمرضى المصابين بأذيات عنق نافذة، و الذين شوهدوا في مشفى الطوارئ في مدينة أربيل في العراق في الفترة 1993-1998.
طريقة الدراسة : أجريت دراسة حالات استعادية (راجعة)، و حللت تبعا لموضع الإصابة في العنق، و الأعراض و طبيعة الإصابة و الاختلاطات و التدبير الجراحي و الوفيات.
النتائج : بلغ عدد المصابين 216 مريضا، بينهم 201 (93 %) مصابين بطلقات نارية و 8 مرضى (3.7 %) مصابين بجروح شظايا و 7 مرضى (3.2 %) مصابين بطعنات بآلات حادة.
كان بين المصابين 180 ذكراً (83.3 %) و 36 أنثى (16.7 %) أي أن نسبة الذكور إلى الإناث بلغت 5 : 1.
تراوحت الأعمار بين 8-60 سنة (وسطي العمر 25.3 سنة).
أجري التدبير الجراحي لدى 100 مريض (46 %)، و توفي 6 مرضى (2.,7 %)، بينهم أربعة مرضى توفوا أثناء و بعد العمل الجراحي لشدة الإصابات، بينما توفي مريضان قبل الجراحة.
كان الإنتان الموضعي أكثر مشاهدة في حال التدبير المحافظ.
الخلاصة إن المعالجة المحافظة آمنة في إصابات العنق النافذة عندما يتم تقييم الحالة قبل الجراحة بشكل جيد مع إجراء التصوير الشعاعي الظليل و التصوير الوعائي في حال توفرهما.
Abstract EN
Objectives : The aim of this study is to describe the presentation, management, and outcome of patients with penetrating injuries of the neck who presented to the Emergency Hospital ofErbil, Iraq between 1993 and 1998.
Methods: Case histories were reviewed and analyzed with regard to region of the neck where the injury occurred, presenting symptoms, nature of injury, complications with and without surgical management, and mortality.
Results : Of the 216 patients, 201 (93 %) had gunshot wounds, 8 (3.

7 %) were female, giving a male to female ratio of 5 : 1 The ages ranged between 8-60 years with a mean age of 25.

Fifty-two patients (24 %) underwent surgical intervention.
There were significant sequelae in 100 patients (46 %).
Of the 6 patients who died, 4 had been treated surgically and two died while being observed before surgery.
Local sepsis was more common in patients treated conservatively.
Conclusion: Non- operative treatment in penetrating neck injuries is safe when comprehensive preoperative evaluation including contrast radiography and angiography is available.
