Background and objectives:Esophageal strictures are common complication following surgical repair of esophageal atresia, corrosive ingestion and reflux esophagitis in children.

There is no current hard evidence supporting usage of balloon catheter pneumatic dilation over bougienage dilation as a treatment of choice for esophageal strictures.

In this study, we are reporting our experience in term of safety and efficacy of utilizing both balloon and bougienage dilation techniques over seven years of treating and following up children with esophageal strictures.

Patients and methods: A retrospective cohort study is achieved at Queen Rania Al Abdullah Hospital for Children which involved all children presented with signs and symptoms of esophageal stricture as a result of previous corrosive ingestion, following surgical repair of esophageal atresia or due to reflux esophagitis during the period from 2011 to 2018.

The studied data included demographics, radiographic and endoscopic diagnosis.

Additionally symptomatic relief, recurrence rate, hospital stay, complications, and post procedural painwere evaluated.

Hundred and two patients are involved in this study.

Seventy three patients were dilated through balloon catheters while another twenty nine patients are dilated utilizing the Savary-Gilliard bougies.

Results of interest were compared between the two groups using the Chi-Square test of goodness-of-fit by IBM-SPSS version 20 software for data analysis.

Results:Seventy three patients are undergone balloon catheter dilation with a mean age of 6.75 (±4.76).

Another Twenty nine patients are undergone Savary-Gilliard bougienage dilation with a mean age of 5.17 (±3.30).

Studied outcomes between the two groups are the number of dilatations needed until the patient becomes symptoms-free which is found to be 4.47 (±1.12) and 6.44 (±1.13) for first and second groups, respectively.

Also, complications following each procedure were found to be 5.4% in balloon catheter pneumatic dilation group where 13.7% for Savary-Gilliard bougienage dilation group.

Recurrence rate within the first year of last dilatation found to be 8.2% for balloon catheter dilation group, in contrast to Savary-Gilliard dilated patients where the recurrence rate is 10.3%.

Chi-Square test of goodness-of-fit was calculated and revealed that balloon catheters are better used for short strictures while SavaryGilliard bougies are better used for long strictures (X2 =4.76, p<0.05).

Conclusion:Balloon catheter dilation is a safe and effective method of treatment for esophageal strictures in children following anastomotic strictures post esophageal atresia repair and post corrosive injuries.

Number of dilations which are needed to relief symptoms and also recurrence stenotic rate are less in balloon catheters usage when it compared to Savary-Gilliard dilators.

Catheter pneumatic dilation is best used when esophageal strictures are short.
