We report a case of a 67-year-old male with unilateral iridocorneal endothelial syndrome in the left eye who subsequently developed a cataract in that eye.
His secondary glaucoma was well controlled by topical beta-blocker therapy and two previous glaucoma surgeries.
Anterior segment visualization was borderline owing to mild corneal haze, iris atrophy, polycoria, and corectopia.
However, we were able to overcome these challenges by judiciously using iris hooks to dilate the pupil, staining the anterior capsule with trypan blue, and employing a soft shell technique to protect the corneal endothelium.
