Background& Aim: Outcomes in pediatric osteosarcoma have dramatically improved over the past few decades, with overall survival rates of 70% and 30% for patients with localized and metastatic disease, respectively.

We aimed to review outcome and prognostic factors that influence overall survival (OS) in osteosarcoma patients at Children’s' University Hospital in Damascus.

Material And Methods: We retrospectively reviewed clinical characteristics and outcomes of 28 patients treated between 2005 and 2015 at a single institution in Syria.

All patients received a uniform chemotherapy regimen consisting of ifosphamide, etoposide, Cisplatin and doxorubicin depending on their tumor stage.

Results: After a follow-up of 60 months, patients with osteosarcoma had 5- year overall and event-free survival rates of approximately 57.1 % and 46.4%, respectively.

The most common primary site was the long bones around the knee (n = 25; 89.2%).9 patient had metastatic disease at diagnosis (8 patients had metastatic disease to lungs, and 1 had metastatic disease to the bones).

In our analysis, adverse prognostic factors were metastases at diagnosis, elevated concentrations of ALP and relapsed disease.

Conclusion: Metastatic disease at diagnosis, elevated concentrations of ALP and relapse have been established as the most powerful prognostic parameters in our analysis.
