هدف الدراسة تقييم العلاقة بين ارتفاع البروتين الشحمي a و حدوث و شدة اعتلال الشبكية عند مرضى النمط الثاني للداء السكري.
أجريت هذه الدراسة على 122 شخصا كان منهم 31 أسوياء ظاهريا و 91 مصابا بالنمط الثاني من الداء السكري (61 منهم مصابون باعتلال شبكية و 30 غير مصابين باعتلال الشبكية)، جرى تشخيص اعتلال الشبكية بتنظير قعر العين و أجريت للمشاركين تحاليل دموية شملت الخضاب السكري و السكر الصيامي والاختبارات الروتينية لبروفايل الشحوم (كولسترول -ثلاثيات أسيل الغليسيرول-HDL- C) والبروتين الشحمي Lp(a) a.
تباينت النتائج في قيم (Lp(a تباينا مهما إحصائيا و كانت القيمة الأعلى عند مجموعة المصابين باعتلال الشبكية، و كان ارتباطه بعوامل الاختطار المدروسة مهما و إيجابيا عدا ارتباطه مع HDL-C الذي كان سلبيا، كذلك وجد ارتفاع مهم إحصائيا في قيم مكونات بروفايل الشحميات في مجموعة السكريين مع اعتلال شبكية عدا HDL-C الذي كان متوسطه في هذه المجموعة أقل من باقي المجموعات (0.001>p).
لم يلاحظ أية علاقة بين شدة اعتلال الشبكية السكري و المكونات الشحمية المدروسة بما فيها (Lp(a.
خدد عيار البروتين الشحمي a 36.1 مغ / دل نقطة الفصل الأفضل للتنبؤ باعتلال الشبكية السكري باستخدام منحنى خصائص تشغيل المستقبل (المسافة تحت المنحني=0.772).
أظهرت النتائج وجود علاقة بين ارتفاع البروتين الشحمي a و حدوث اعتلال الشبكية السكري في مرضى النمط الثاني للداء السكري مما يرجح اعتباره عامل اختطار لحدوث هذا المرض
Abstract EN
he aim of this study was to determine the association between high lipoprotein a [Lp(a)] and diabetic retinopathy (DR) incidence and severity in type 2 diabetes mellitus.
The study included 122 participants, 31 were healthy, 30 had type 2 diabetes mellitus (T2DM) without DR, and 61 had DR.
Retinopathy was assessed by fundoscopy.
Lp(a), lipid profile component [cholesterol, triacylglycerol, high density lipoprotein cholesterol (HDL-C)], fasting glucose, glycated haemoglobin (HbA1c) were measured.
Lp(a), lipid profile components were significantly higher in DR group compared to the other groups, except for HDL-C which was lower in DR group (P<0.001).
Lp(a) exhibited significant positive correlations with other risk factors; however, it had a negative correlation with HDL-C.
No relationship was observed between Lp(a), lipid profile components and DR severity.
The cut-off value of Lp(a) assessed by receiver operating characteristic curve was 36.1 mg/dl (urea under curve 0.772) for predicting diabetic retinopathy.
These findings showed an association between high Lp(a) serum concentrations and DR incidence in T2DM patients which suggests that Lp(a) is a risk factor for DR.
