Meigs’ syndrome is rare.

it is described as a triad of solid benign ovarian tumor, ascites, and pleural effusion that resolve after tumor resection.

We present a case of a 36-year-old Bahraini female who presented with signs and symptoms of acute pelvic pain and adnexal torsion.

She had left ovarian fibrothecoma with elevated CA125, moderate ascites and right-sided pleural effusion.
