الورم الميلانيني أو الجلدي الشرجي هو ورم نادر يمثل أقل من من السرطانات الشرجية و 3.().7 من الأورام الميلانينية الخبيثة.
إن فظيع بسبب ظهور انتشار الورم مبكرا.
قمنا بدراسة حالتين في هذا الخصوص.
الحالة الأولى: امرأة مصابة بارتفاع ضغط الدم تبلغ من العمر 78 عاما و التي أبلغتنا عن حصول نزيف شرجي ظهر لها من مدة أسبوع دون اضطرابات في عبور الأمعاء أو تدهور الحالة العامة.
أظهر فحص المستقيم أن الورم التقرحي الناشئ للمظهر الأسود يقع على مسافة 3 سم من الهامش الشرجي الذي امتد لأكثر من 7 سم.
خلص الفحص المرضي إلى أن الورم هو الورم الميلانيني الشرجي مع نتائج إيجابية لكيمياء المناعة المرضية للأجسام المضادة ل Melan-A و.Anti-HMB45 الحالة الثانية: امرأة مصابة بارتفاع ضغط الدم تبلغ 76 عاما و التي استشارت لحصول نزيف المستقيم المتكرر المرتبط بمتلازمة المستقيم والتدهور الأخير في الحالة العامة.
كشف فحص المستقيم عن ورم متقرح مسود في قطر 3سم يقع على بعد 2 سم من الهامش الشرجي.
وخلص الفحص المرضي إلى أن الورم من النوع الميلانيني الشرجي مع كيمياء المناعة المرضية الإيجابية للأجسام المضادة ل Melan-A ومضادات البروتين.100 S لم يظهر التقييم الإرشادي للمرض حصول انتشار موضعي أو بعيد في كلتا الحالتين .
تمثل العلاج في استئصال محلي للورم لدى المريضتين و اللتين فقدتا بعد ذلك من أجل المتابعة.
Abstract EN
Anorectal melanoma is a rare tumor, representing less than 1% of anorectal cancers and 0.3% of malignant melanomas.
Its prognosis is poor because of the early onset of metastases.
We report two cases.
First case: 78-year-old hypertensive woman with rectal bleeding evolving for a week without transit disorders or deterioration of the general condition.
Proctological examination had shown an ulcerovegetative tumor of brownish appearance located 3cm from the anal margin, which extends over 7cm.
The pathology examination concluded to anorectal melanoma with positive immunohistochemistry for anti-Melan-A and anti-HMB45 antibodies.
Second case: 76-year-old hypertensive woman who consulted for recurrent rectal bleeding with rectal syndrome and recent deterioration in general condition.
The proctological examination found a brownish ulcerovegetative tumor of 3cm in diameter located 2cm from the anal margin.
The pathology examination concluded to anorectal melanoma with positive immunohistochemistry for the anti-Melan-A and anti-protein S100 antibodies.
The extension assessment did not show locoregional or distant metastases in both cases.
The therapeutic gesture was a local excision for the two patients who were then lost to follow-up.

Le mélanome anorectal est une tumeur rare, représentant moins de 1 % des cancers anorectaux et 0,3% des mélanomes malins.
Son pronostic est redoutable à cause de la survenue précoce de métastases.
Nous en rapportons deux observations.
1 er cas: femme âgée de 78 ans, hypertendue, qui présentait des rectorragies évoluant depuis une semaine sans troubles de transit ni altération de l’état général.
L'examen proctologique avait montré une tumeur ulcéro-bourgeonnante d'aspect noirâtre située à 3cm de la marge anale qui s'étendait sur 7cm.
L'examen anatomopathologique avait conclu à un mélanome anorectal avec positivité à l’immunohistochimie pour les anticorps anti Melan-A, et anti HMB45.
2 ème cas: femme âgée de 76 ans, hypertendue, qui consultait pour des rectorragies récidivantes associées à un syndrome rectal et une altération récente de l'état général.
L'examen proctologique avait mis en évidence une tumeur ulcérobourgeonnante noirâtre de 3cm de diamètre située à 2 cm de la marge anale.
L'examen anatomopathologique avait conclu à un mélanome anorectal avec positivité à l’immunohistochimie pour les anticorps anti Melan-A et anti-proteine S100.
Le bilan d'extension n'a pas objectivé de métastases locorégionales ou à distance dans les deux cas.
Le geste thérapeutique était une exérèse locale pour les deux malades qui ont été ensuite perdues de vue.
