Allergen immunotherapy (AIT) is the only etiological treatment of allergic disorders particularly allergic rhinitis and asthma.

Its principle is based on obtaining an immunologic tolerance following the administration of increasing quantities of allergenic extracts.

There are 2 main types of AIT : subcutaneous immunotherapy and sublingual immunotherapy.

The protocol for using AIT depends on the route of administration, the seasonal or perennial nature of allergic rhinitis and the laboratory recommendations.

AIT is indicated for moderate to severe allergic rhinitis, intermittent or persistent (seasonal or perennial), not controlled by symptomatic treatments.

The physician must inform the patient about the principle of AIT, the duration, the protocol and especially the cost and side effects, the most serious of which is anaphylactic reactions.

The efficacy of AIT has been proven in the treatment of allergic rhinitis.

AIT also has a preventive effect : prevention of the appearance of asthma or new sensibilizations.

At present, sublingual immunotherapy is widely favoured over subcutaneous immunotherapy because of its similar efficacy, ease of use and a lower risk of serious complications.

L'aménorrhée hypothalamique fonctionnelle est une forme d'anovulation chronique souvent en relation avec un stress intense, une perte de poids, un exercice excessif ou la combinaison de ces différents facteurs.

Le diagnostic ne peut être porté qu'après exclusion des autres causes d'aménorrhée.

L'évaluation se fait chez des femmes dont l'intervalle entre les règles excède 45 jours de manière persistante ou en cas d'aménorrhée > 3mois.

En cas d'aménorrhée primaire, une imagerie des voies génitales est indispensable.

Sur le plan thérapeutique, une prise en charge nutritionnelle visant à reprendre du poids est indispensable ainsi qu'un soutien psychologique, de préférence en thérapie comportementale.

En cas d'échec, il est recommandé de prescrire un traitement comportant des estrogènes transdermiques associés à des progestatifs oraux plutôt que des contraceptifs oestroprogestatifs.

En cas de désir de grossesse, un traitement par pompe à GnRH est indiqué en première ligne.
