الهدف : الهدف من الدراسة الحالية هو تحديد الدور الذي تلعبه مظاهر القلق و الإكتئاب و الضغوط النفسية في الإصابة بمرض إرتفاع ضغط الدم، في المجتمع القطري.
الخطة : هي دراسة إسترجاعية للمقارنة بين أشخاص مرضى و آخرين أصحاء.
الموقع : تم إجراء الدراسة في مراكز الرعاية الصحية الأولية.
الطريقة : تم إجراء الدراسة خلال الفترة من يناير 2003-يوليو 2004، على مجموعة من المواطنين القطريين تتراوح أعمارهم بين 25-65 سنة.
شملت الدراسة 800 شخص، منهم 400 شخص مصاب بإرتفاع ضغط الدم و 400 شخص سليم.
فقط 318 من مجموع المصابين بالمرض، و 316 من مجموع الأصحاء إستجابوا للإستبيان، و ذلك بمعدل إستجابة بلغ 79.3%.
تعريف مرض إرتفاع ضغط الدم وفق معايير منظمة الصحة العالمية : ضغط دم إنقباضي (SBP) ≥ 140 mmHg و ضغط دم إنبساطي (DBP) ≥ 90 mmHg.
إشتملت هذه الدراسة على إستبيان، يجرى وجهاً لوجه مع الأشخاص موضوع الدراسة، و يدور حول مدى تأثير مظاهر القلق و الإكتئاب و الضغوط النفسية في الإصابة بمرض إرتفاع ضغط الدم.
النتائج : من مجموع المرضى الذين أجريت عليهم الدراسة، بلغت نسبة الرجال 42.1%، بينما بلغت نسبة النساء 57.9%.
المتوسط ± الإنحراف المعياري، للرجال مقابل النساء يساوي (45.7 ± 14.6 و 41.3 ± 13.1 على التوالي، p < 0.001).
أظهرت الدراسة الحالية أن العمر، و السمنة BMI > (30)، و مرض السكري، و عادة التدخين، و الضغط النفسي، و القلق، و الإكتئاب كلها تعتبر عوامل خطر للإصابة بإرتفاع ضغط الدم، و قد تم تأكيد هذه النتائج بواسطة العديد من التحاليل.
فضلاً عن ذلك أثبتت الدراسة أن هناك علاقة إيجابية بين إرتفاع ضغط الدم و الإضطرابات النفسية و القلق و الإكتئاب و الضغوط النفسية.
الخلاصة : أثبتت الدراسة الحالية أن القلق و الإكتئاب و الضغوط النفسية لها علاقة بإرتفاع ضغط الدم.
Abstract EN
Objective : The aim of the present study was to examine the role of stress, anxiety and depression symptomatology in the development of hypertension in Qatari population.
Design : This is a retrospective matched case-control study.
Setting : The survey was carried out at the Primary Health Care Centers (PHCs).
Methods : The survey was conducted from January 2003 to July 2004 among Qatari nationals 25 to 65 years of age.
During the study period, a total of 800 subjects, 400 cases and 400 controls, were approached.
Of whom, 318 cases and 316 controls responded to the questionnaire, for a response rate of 79.3%.
Hypertension was defined according to WHO criteria as systolic blood pressure (SBP) > 140 mmHg and/or diastolic blood pressure (DBP) > 90 mmHg.
This study includes a questionnaire based on face-to-face interview on stress, anxiety and depression symptoms.
Results : Of the total number of patients surveyed, 42.1% were males and 57.9% females.
The mean ± SD age (in years) for males versus females was (45.7 ± 14.6 and 41.3 ± 13.1, respectively).
The present study showed that age, obesity (BMI > 30), diabetes, smoking habits, stress, anxiety and depression can be considered as risk factors for hypertension as discovered using multivariate analysis.
Furthermore, the study suggests there is a positive correlation between hypertension and psychiatric disorders, anxiety, depression and stress.
Conclusion : The present study suggests that anxiety, depression and stress are related to hypertension.
