تعتمد معالجة الالتهابات المزمنة للأنسجة حول السنية على إزالة العامل الرئيسي المسبب لها و هي اللويحة البكتيرية (طبقة بلاك) و التي تتواجد عادة فوق و تحت اللثة مسببة تشكل الجيوب حول السنية و امتصاص العظم و من ثم فقد الدعم السني.
و لقد اعتمدت هذه الدراسة على استئصال الجيوب حول السنية العميقة و تسوية الجذور و من ثم تطبيق مادة الأتريدوكس الموضعية على جانب واحد فقط من الفم و مقارنته بالطرف الآخر.
تم إجراء هذه الدراسة على (15) خمسة عشر مريضا من المرضى الذين يعانون من التهابات الأنسجة حول السنية المتوسطة و المتقدم و تم التأكد من خلوهم من الأمراض العامة و عدم تلقيهم لأي معالجة لثة منذ ستة أشهر و يهدف هذا البحث إلى التقييم الإكلينيكي و البكتيري لتأثير التطبيق الموضعي لمادة الأتريدوكس (دوكس سيكلين جل) في معالجة الالتهاب المزمن للأنسجة حول السنية مرة واحدة في فترات زمنية مختلفة كل ثلاثة أشهر و لمدة سنة.
• تم فحص الإكلينيكي لكل مريض لمعرفة حالة اللثة و الأنسجة حول السنية و ذلك قبل و بعد المعالجة.
• تم أخذ عينة من عمق الجيوب اللثوية الخاضعة للمعالجة اللثوية في كلا الجانبين قبل و بعد المعالجة.
و قد أظهرت النتائج المستخلصة من هذه الدراسة ما يلي : • هناك تحسن و انخفاض في اللويحة الجرثومية (البلاك) و التهابات اللثة و عمق الجيوب حول السنية في كلا الجانبين.
• هناك فروق في النتائج الإكلينيكية بين الجانبين و لكنها ضئيلة و ذلك لصالح الجهة التي طبق فيها الدواء الموضعي.
• أظهرت النتائج البكتيرية أن هناك تأثير فعال للدواء الموضعي على البكتيريا المسببة للالتهابات حول السنية أكثر من تأثير عملية إزالة الجيوب و تسوية الجذور فقط.
Abstract EN
Objective : The present study was designed to evaluate clinically and microbiologically the effects of topical application of CHLO-SITE (Chlorohexidine gel) in management of chronic periodontitis.
Subjects and Methods : This study was carried out on 15 patients (aged 25-55) with chronic periodontitis.
They were received scaling and root planning (SRP) alone in one side and SRP plus CHLO-SITE (Chlorhexidine gel) in other side.
Each individual was subjected to the following measurements; (1) Evaluation of the clinical parameters pre and post treatment to detect the outcome of the treatment modality, (2) Dental plaque samples initially and at 3, 6, 9 and 12 months were obtained for microbiological evaluation.
Results showed that (1) CHLO-SITE (Chlorhexidine gel) delivered locally into periodontal diseased sites reduced all sub gingival bacteria and (2) Both treatment and modality led to a highly statistically significant reduction in microbiological counts as well as clinical parameters applied.
No clinical relevant side effects were observed.
