الهدف من هذه الدراسة هو تقييم العلاقة بين بطانة جدار الرحم المبينة بواسطة الموجات فوق الصوتية و بنتائج الحمل عن طريق أطفال الأنابيب.
و قد تمت عملية إخصاب مساعد و أطفال أنابيب لعدد 66 حالة، كما تم استخدام 66 حالة أخرى عولجت عن طريق الأجنة المجمدة من دورات سابقة لنفس المرضى.
و قد تم إجراء أشعة الموجات فوق الصوتية عن طريق المهبل لجميع الحالات المشتركة في هذا البحث لمعرفة سمك و طبيعة بطانة جدار الرحم بعد ثلاثة أيام من حقن الأجنة و إعادتها داخل الرحم.
و تم تقسيم حالات الدراسة إلى مجموعتين، المجموعة الأولى و بها بطانة الرحم متناسقة و عالية المردود الصوتي، و كان نعدل الحمل 31 % مقابل 14 % في المجموعة الثانية و بها بطانة الرحم غير متناسقة و عالية المردود الصوتي.
و كان معدل الإجهاض في المجموعة الأولى 21 % مقابل 33 % في المجموعة الثانية و هو ذو دلالة إحصائية.
و نستنتج من هذه الدراسة أن وجود النمط المتناسق عالي المردود الصوتي لبطانة الرحم ثلاثة أيام بعد حقن الأجنة يعتبر ذو دلالة هامة على نتائج الحمل و الولادة لحالات أطفال الأنابيب.
Abstract EN
Backgrounds : Successful in-vitro fertilization (IVF) outcome depends on the endometrial receptivity at the time of implantation.
Objectives : To evaluate the relationship between the sonographic assessment of the endometrial in the lacteal phase and the in-vitro fertilization embryo transfer outcome.
Design : Prospective observational study.
Setting : Bishop Auckland General Hospital, County Durham, UK Patients and Methods : Sixty-six women undergoing IYP-ET and another 66 women undergoing frozen embryo transfer (FET).
All women underwent transvaginal ultrasound evaluation of the endometrial thickness and acrogenic pattern three days after their embryo transfer.
The 132 evaluated cycles were classified into two groups; those with homogeneous hyperechogenic (HH) endometrial pattern and those with non-homogeneous hyperechogenic pattern (non-HH).
Results : The clinical pregnancy rates were 31 % versus 14 % (P < 0.05), the live birth rates were 24 % versus 9 % (P < 0.05), the implantation rates were 14 % versus 4 % (P < 0.05) and the miscarriage rates were 21 % versus 33 % (P < 0.05) in HH and non-HH cycles in IVF-ET group.
There were no statistical differences between the pregnancy outcomes and echo pattern in the FET group of women.
There were no significant differences in age, number of oocytes retrieved, fertilization rate, or number of embryos transferred between conception and non-conception cycles in IVF-ET.
Conclusion : The presence of (HH) endometrial pattern 3 days after ET is a good predictor of IVF-ET outcome than in FET cycles.
