إن العلاج الجراحي القياسي لأورام الغدة النكافية يتطلب إما استئصال سطحي، أو استئصال كلي للغدة مع المحافظة عل العصب السابع (الوجهي)، و بالرغم من أن هذه العملية الجراحية فعالة في تقليل الارتجاع، إلا أن التشريح الاعتلالي للعصب السابع قلما ينظر إليه.
و في هذه الدراسة تم مراجعة 23 حالة تعاني من أورام الغدة النكافية الحميدة و الخبيثة و قد أجريت لهذه الحالات عمليات استئصال جزئي أو كلي مع المحافظة على العصب السابع و طلك بمستشفى باب الشعرية الجامعي حيث تم الالتفات إلى تقييم وظيفة العصب السابع بعد العملية و مدى تأثره.
و قد بلغت نسبة الاختلال الوظيفي العابر للعصب السابع 33.3 % في حالات الاستئصال الجزئي، 40 % في حالات الاستئصال الكلي بنسبة كلية 34.8 % و أن نسبة الاختلال الوظيفي المستديم كان أقل في حالات الاستئصال الجزئي 5.5 % و أعلى في الاستئصال الكلي للغدة 20 %.
و قد لوحظ أن حدوث الاختلال الوظيفي المستديم للعصب السابع أعلى في حالات المرتجعة و في وجود الأورام الخبيثة و كذلك في حالات الاستئصال الكلي للغدة.
Abstract EN
Standard surgical management for tumors of the parotid gland requires superficial, subtotal, or total parotidectomy with preservation of the facial nerve.
Although this approach is effective in minimizing recurrence, the resultant facial nerve morbidity is seldom addressed.
Twenty three consecutive patients who underwent parotid surgery for benign or malignant neoclassic at Babb El-Shieria university hospital are reviewed, with attention to postoperative facial nerve function.
Transient dysfunction was frequently encountered in superficial and total parotidectomy (33.3 %, 40 % respectively) with total incidence of 34.8 %.
But permanent dysfunction was uncommon in superficial parotidectomy group (5.5 %), although it was higher in total parotidectomy group (20 %).
The incidence of long-term dysfunction was higher in recurrent cases and when total parotidectomy was performed also when malignant tumors were encountered.
